CAUSE/ CORRECTIVE ACTION / COMMENTS

1. At 0206 on 980817, the unit was shut down due to IB steam generator tube leakage. Reference Voluntary LER 98-002-00 (Unit 1) dated 980903. Inspection and tube plugging in 1B steam generator was completed.

The unit was synchronized to the grid at 0706 on 980904. The unit was returned to 100% reactor power at 0133 on 980907.

2. At 0516 on 980909, the unit experienced a turbine trip causing a consequent automatic reactor trip due to a card failure on the Solid State Reactor Protection System. The failed card was replaced, and the unit was synchronized to the grid at 0944 on 980910. The unit was returned to 100% reactor power at i146 on 980812. Reference LER 98-004-00 (Unit 1) dated 980928.

NARRATIVE REPORT The forced outage due to 1B steam generator tube leakage was completed. All identified 1B steam generator tube leakage was plugged, and 'he unit was returned to power operation.

The unit also experienced a turbine trip causing a consequent automatic reactor trip due to a card failure on the Solid State Reactor Protection System. The failed card was replaced and the unit was returned to power operation.

CAUSE/ CORRECTIVE ACTION / COMMENTS NARRATIVE REPORT At 2325 on 980925, with the unit operating at 100% reactor power, the unit was ramped to 15% reactor power to perform a chemistry flush of the steam generator. Flushing was completed and the unit was rctumed to 100% reactor power at 1153 on 980928.
