Dear Mr. O'Reilly:
This is to confirm the telephone call to the Region II hPC office by Mr.
D. Morey (APCo) on September 17, 1983 that was made in accordance with Unit 2 Technical Specification 6.9.1.12.e.
At 1635 on September 17, 1983,[while attempting to isolate steam to the Unit 2 Turbine Building, three Main Steam Isolation Valves (two loop B and one loop C) failed to close under no flow conditions] The loop C valve and one loop B valve were fully closed at 1711 and 1715, respectively by using the valve test switches.
Although the other loop B valve could be partially closed, it could not be fully closed.
The loop B valve bonnet has been removed and the clapper was found de-tached from the valve arm with a mounting bolt nut laying on the arm.
Apparently the clapper became loose during plant operation and subsequently detached from the arm during the attempts to close the valve af ter plant shutdown.
Unit 2
of Farley Nuclear Plant is currently undergoing a
refueling / maintenance outage and the investigation of this incident will continue.
