CAUSE/ CORRECTIVE ACTION / COMMENTS NARRATIVE REPORT The following refueling outage activities were completed:

1) Steam Generator eddy current testing and sleeving
2) Baffle former bolt inspection and replacement
3) Power uprate design change
4) Incore thimble replacement On 981228 at 2223 the UlRF15 outage was corapleted and the unit was synchronized to the grid.

On 981228 at 2313 the main condenser lost vacuum and tripped the turbine.

Condenser vacuum was restored and the unit was sychronized to the grid on 981229 at 0245.
