This Special Report is being submitted in accordance with Technical Specification (TS) 3.3.3.1, Action Statement 22A, to inform the NRC that the Containment 650'levation High Range Train. 'A'rea Radiation Detector, 2-VRA-2310, was inoperable longer than allowed by the Action Statement.
ThefailuretocomplywiththeSpecialReportrequirement wasdiscovered onJuly30,1993,andisrectified bytransmittal ofthisletter.Thiseventresultedfromtherepairactionnotbeingprioritized inaccordance withtheapplicable LCOrestoration requirement, coincident withthefailuretoidentifytheneedforaSpecialReport.Thisisattributed to:1)ineffective communication oftheTSActionStatement requirement viatheWorkRequest,.
On July 10, 1993 at 1330 hours, the detector was declared inoperable due to the inability to perform the channel checks required by the surveillance requirements.             Although the detector was          still functional, it was considered and declared inoperable. The redundant instrumentation was confirmed to be operable.

A  hard reboot was performed, the parameter files were inserted, and the channels were reinitialized. The Surveillance 2 IHP, 4030 STP.195 was performed satisfactorily and the detector was declared operable on July 29, 1993 at 1430 hours.
To prevent this problem from recurring, actions are being taken by the Plant to improve the Action Request initiation and review process.

This Special Report is being submitted in accordance with Technical Specification (TS) 3.3.3.1, Action Statement 22A, to inform the NRC that the Containment 650'levation High Range Train. 'A'rea Radiation Detector, 2-VRA-2310, was inoperable longer than allowed by the Action Statement.

On July 10, 1993 at 1330 hours0.0154 days <br />0.369 hours <br />0.0022 weeks <br />5.06065e-4 months <br />, the detector was declared inoperable due to the inability to perform the channel checks required by the surveillance requirements. Although the detector was still functional, it was considered and declared inoperable. The redundant instrumentation was confirmed to be operable.

However, the Special Report required by TS 3.3.3.1 was not submitted to the NRC when the detector remained inoperable in excess of the 7 day Action Statement requirement.requirement The failure to comply with the Special Report was discovered on July 30, 1993, and is rectified by transmittal of this letter.
