Missed Surveillance - DC Batteries-and Chargers.

This report was initiated by Incident Report 82-342.

CONDITIONS PRIOR TO OCCURRENCE:

Unit 1 - Mode 1 - Rx Power 100% - Unit Load 1120 MWe, Unit 2 - Mode 1 - Rx Power 82% - Unit Load 910 MWe.

. DESCRIPTION OF OCCURRENCE:

On October 6, 1982, following satisfactory completion of Maintenance Procedure M3M, it was discovered that the: procedure had not been performed the preceding week as required by Technical Specification Surveillance Requirements 4.8.2.3.2.a, and 4.8.2.5.2.a.

The surveillance is required to demonstrate the' operability of Salem Generating Station Units 1 and 2 28VDC and 125VDC batteries and j chargers.

p DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

i The surveillance was missed because its assignment to a station electrician was inadvertently overlooked by the electrical super-i visor. Scheduling of the procedure has not been incorporated into the station Inspection Order system due to-the delay involved in generating a work order to initiate performance of the procedure.

ANALYSIS OF OCCURRENCE:

The operability of DC power sources insures that power will be available to supply safety related equipment required for safe shutdown of the facility and mitigation of accidents. considered

in the FSAR. As noted, the DC batteries and chargers tested' l satisfactorily on the following surveillance, indicating'that i they were operable.throughout the occurrence. As such, the

! incident involved no risk to the health or safety of the public.

! The occurrence constituted an inadequacy in the implementation i

of administrative controls which threatened to cause a reduction

of the redundancy provided in reactor protective and engineered safety feature systems. The-incident is therefore reportable in accordance with Technical Specification 6.9.1.9.c.

  • ANALYSIS OF OCCURRENCE: (continued)

Surveillance Requirements 4.8.2.3.2.a, and 4.8.2.5.2.a require:

Each battery and charger shall be demonstrated operable by at least once per 7 days verifying that:

1. The electrolyte level of the pilot cell is between the minimum and maximum level indication marks.
2. Thg pilot cell specific gravity, corrected to 77 F and full electrolyte level, is 3 1.200.
3. The pilot cell voltage is 3 2.08 volts.
4. The overall battery voltage is 3 27 volts or 125 volts, as applicable.

CORRECTIVE ACTION:

To prevent future problems of this nature, a monthly Inspection Order card was written to generate weekly work orders for the surveillance. A supervisor was designated to write the work orders and track completion of the monthly requirement.
