Containment-Plant Vent Radioactivity Monitor - Inoperable.

This report was initiated by Incident Report 82-121.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 - Rx Power 99% - Unit Load 1120 MWe.

DESCRIPTION OF OCCURRENCE:

On May 8, 1982, during routine operation, the Containment-Plant Vent Radioactivity Monitor Lcw Flow Alarm was received.

The particulate and gaseous radioactivity channels were de-clared inoperable, and at 0316 hours0.00366 days <br />0.0878 hours <br />5.224868e-4 weeks <br />1.20238e-4 months <br />, Limiting Condition for Operation 3.3.3.1 Action 24 and Action Statement 3.4.7.1 were entered.

This occurrence constituted operation in a degraded mode in accordance with Technical Specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Valve 2VC908 had drifted partially closed due to a loosened packing, restricting monitor sample flow.

ANALYSIS OF OCCURRENCE:

Technical Specification 3.3.3.1.b Action 24 requires:

With the number of operable channels less than the minimum number of operable channels required, perform area surveys of the monitored area with portable monitoring instrumentation at least once per 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br />.

Technical Specification 3.4.7.1 requires:

With only two of the required leakage detection systems operable, operation may continue for up to 30 days provided grab samples of the containment atmosphere are obtained and analyzed at least once i per 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> when the required gaseous and/or par-  !

CORRECTIVE ACTION:

An investigation revealed that Valve 2VC908 packing had loosened, and system piping vibration caused the valve to drift in the closed direction. The packing nut was properly tightened, and Valve 2VC908 was re-opened. Sample flow was returned to. normal, and the radioactivity monitors were satisfactorily tested. The containment-plant vent monitor was declared operable at 1454 hours0.0168 days <br />0.404 hours <br />0.0024 weeks <br />5.53247e-4 months <br />, May 8, 1982, and Limiting Condition for Operation 3.3.3.1 Action 24 and Action Statement 3.4.7.1 were terminated.
