No. 21 Steam Generator Level Channel 2 - Inoperable.

This report was initiated by Incident Report 82-032.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 - Rx Power 96% - Unit Load 1090 MWe DESCRIPTION OF OCCURRENCE:

At 1030 hours0.0119 days <br />0.286 hours <br />0.0017 weeks <br />3.91915e-4 months <br />, February 9, 1982, No. 21 Steam Generator Level Channel 2 displayed erratic behavior, rapidly drifting high to 65%, causing feed flow to drop, dropping actual level to 30%.

The operator immediately switched the feed regulator valve to manual control and regained proper steam generator level.

Channel 2 drifted back to normal.

A strip recorder was hooked up to the channel and noise discovered on the signal lead.. The problem was assumed to be in the transmitter, and it was isolated in preparation for replacement.

The channel was declared in-operable, and at 2310 hours0.0267 days <br />0.642 hours <br />0.00382 weeks <br />8.78955e-4 months <br />, Limiting Condition for Operation-3.3.1 Action 7 was entered.

This occurrence constituted ope 2 stion in a degraded mode in' accordance with Technical Specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The transmitter was replaced, but the noise persisted.

During further investigation for the source of the noise, the signal lead shields were alternately lif ted at the terminal strip junction between the transmitter and instrument.

In both cases, the noise was eliminated by lifting the lead.

The problem re-appeared shortly after. replacing the lead to the. transmitter;.

the noise was permanently eliminated when the lead to the_instru-ment was replaced.

The problem was therefore assumed to involve-an intermittant ground on the signal. lead at the terminal strip; lifting and replacing the shield connections during troubleshooting apparently' eliminated the-problem..

ANALYSIS OF OCCURRENCE:

Technical Specification 3.3.1 Action 7 requires:

With the number of operable channels one less than the total number of channels, startup and/or operation may proceed until performance of the next required channel functional test provided the inoperable channel is placed in the tripped condition within one hour.

CORRECTIVE ACTION

The channel bistable was placed in the tripped condition within one hour.

With the noise eliminated, the channel was returned to operation, recalibrated and satisfactorily tested.

No. 21 Steam Generator Level Channel 2 was declared operable at 0131 hours0.00152 days <br />0.0364 hours <br />2.166005e-4 weeks <br />4.98455e-5 months <br />, February 10, 1982, and Action Statement 3.3.1 Action 7 was terminated.
