Containment Service Water Leak - No. 21 Containment Fan Coil Unit.

This report was initiated by Incident Report 82-134.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 - Rx Power 100% - Unit Load 1150 MWe.

DESCRIPTION OF OCCURRENCE:

On May 26, 1982, during routine operation, the Control Room Operator observed that No. 21 Containment Fan Coil Unit (CFCU) tripped after being started in high speed. Shortly after, the Containment Sump Overflow annunciator alarmed, and the operator determined the containment sump pump had been running for approximately half an hour. An inspection of containment was made, and a 100 GPM service water leak from No. 21 CFCU motor cooler was discovered. The CFCU was isolated, and leakage was stopped. The unit was declared inoperable at 1450 hours0.0168 days <br />0.403 hours <br />0.0024 weeks <br />5.51725e-4 months <br />, and Action Statement 3.6.2.3.a was entered. In accordance with NRC IE Bulletin 80-24, the NRC was notified by telephone at 1505 hours0.0174 days <br />0.418 hours <br />0.00249 weeks <br />5.726525e-4 months <br />, with written confirmation transmitted on May 27, 1982.

This occurrence constituted abnormal degradation of the containment boundary in accordance with Technical Specification 6.9.1.8.c.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The service water leakage was from a failed motor cooler seam weld. The fan motor insulation was damaged by the leakage, resulting in an overcurrent trip condition.

LER 82-039/0lT l ANALYSIS OF OCCURRENCE:

Technical Specification 3.6.2.3.a requires:

With one group of containment cooling fans inoperable and both containment spray systems operable, restore the inoperable group of cooling fans to operable status within 7 days, or be in at least hot standby within the next 6 hours6.944444e-5 days <br />0.00167 hours <br />9.920635e-6 weeks <br />2.283e-6 months <br /> and in cold shutdown within the following 30 hours3.472222e-4 days <br />0.00833 hours <br />4.960317e-5 weeks <br />1.1415e-5 months <br />.

NRC IE Bulletin 80-24 requires:

That containment service water leaks be considered as as abnormal degradation of the containment boundary.

CORRECTIVE ACTION:

The failed motor cooler was properly replaced; however, attempts to dry out the fan motor failed. The fan motor was properly replaced, and the unit was satisfactorily tested. No. 21 CFCU was declared operable at 1336 hours0.0155 days <br />0.371 hours <br />0.00221 weeks <br />5.08348e-4 months <br />, June 2, 1982, and Action Statement 3.6.3.2.a was terminated.
