Valve 14SW65 - Containment Service Water Leak.

This report was initiated by Incident Report 82-068.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 4 - Rx Power 0% - Unit Load 0 MWe, DESCRIPTION OF OCCURRENCE:

At 2330 hours0.027 days <br />0.647 hours <br />0.00385 weeks <br />8.86565e-4 months <br />, April 3, 1982, while performing a visual in-spection of the Containment Building, an operator discovered a service water leak on the flange of Valve 14SW65.

The leak rate was deterri. sed to be 0.5 GPM.

In accordance eith NRC IE Bulletin 80-24, the NRC was notified of the service water leak in containment by telephone, with written confirmation transmitted on April 5, 1982.

This occurrence constituted abnormal degradation of the containment boundary in accordance with Technical Specification 6.9.1.8.c.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The cause of the leakage was loose studs on the valve flange.

ANALYSIS OF OCCURRENCE:

NRC IE Bulletin 80-24 requires:

That a service water leak inside containment be treated as a degradation of the containment boundary in accordance with Technical Specification 6.9.1.8.c.

CORRECTIVE ACTION

The studs on the valve flange were torqued and the leakage stopped.
