No. 22 Containment Fan Coil, Unit - Inoperable.

This report was initiated by Incident Report 82-107.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 - Rx Power 90% - Unit Load 1040 MWe.

DESCRIPTION OF OCCURRENCE:

On May 1, 1982, during routine surveillance, the Control Room Operator observed zero service water flow to No. 22 Containment Fan Coil Unit (CFCU).

No. 22 CFCU was declared inoperable, and at 1700 hours0.0197 days <br />0.472 hours <br />0.00281 weeks <br />6.4685e-4 months <br />, Action Statement 3.6.2.3.a was entered.

The unit was restored to an operable status and placed in service in high speed.

On May 2, 1982, the operator noticed that service water flow to No. 22 CFCU was 300 GPM less than is required by the Technical Specifications.

The unit was declared inoperable for a second time at 0630 hours0.00729 days <br />0.175 hours <br />0.00104 weeks <br />2.39715e-4 months <br />, and Action Statement 3.6.2.3.a re-entered.

These occurrences constituted operation in a degraded mode in accordance with Technical Specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The zero flow to No. 22 CFCU was due to Valve 22SW57 being stuck in the closed position.

In the second instance, the nut holding the positioner arm to the valve stem was loose, resulting in an improper valve position.

ANALYSIS OF OCCURRENCE:

Technical Specification 3.6.2.3.a requires:

With one group of containment cooling fans inoperable and both containment spray systems operable, restore the in-operable group of cooling fans to operable status within 7 days or be in at least hot standby within the next 6 hours6.944444e-5 days <br />0.00167 hours <br />9.920635e-6 weeks <br />2.283e-6 months <br /> and in cold shutdown within the following 30 hours3.472222e-4 days <br />0.00833 hours <br />4.960317e-5 weeks <br />1.1415e-5 months <br />.

CORRECTIVE ACTION

In the first case, Va3ve 22SW57 was exercised and the CFCU was satisfactorily' tested.

The unit was declared operable at 0500 hours0.00579 days <br />0.139 hours <br />8.267196e-4 weeks <br />1.9025e-4 months <br />, May 2, 1982, and Action Statement 3.6.2.3.a was terminated.
