No. 1C Safeguards Equipment Control System - Inoperable.

This report was initiated by Incident Report 82-062.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 5 - Rx Power 0% - Unit Load 0 MWe.

DESCRIPTION OF OCCURRENCE:

On March 27, 1982, while performing Surveillance Procedure SP ( O) 4 . 3. 2.1.1. a , 4KV Breaker 1C4D failed to remain closed as required. At 2057 hours0.0238 days <br />0.571 hours <br />0.0034 weeks <br />7.826885e-4 months <br /> No. 1C Safeguards Equipment Control (SEC) System was declared inoperable.

Subsequent troubleshooting revealed a temporary jumper installed in the breaker control circuit; the jumper was not documented per Administrative Procedure AP-13. Prior to this date, procedure SP (O) 4. 3.2.1.1.a did not require a check of the operation of the 4KV vital bus supply breakers.

This occurrence involved inadequacies in the implementation of administrative and procedural controls in accordance with Technical Specification 6.9.1.9.c.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The cause of tSis occurrence was an undocumented temporary jumper in Breaker 1C4D control circuit.

ANALYSIS OF OCCURRENCE:

No. 1C SEC being inoperable threatened to reduce the degree of redundancy provided in engineered safety feature systems in accordance with Technical Specification 6.9.1.9.c.

During the period that No. 1C SEC was inoperable, protection was provided by the two operable channels. The jumper, by it-self, would not have resulted in the loss of safeguards equip-ment during an accident. Therefore, this occurrence did not re-sult in any risk to the public or environment.

D CORRECTIVE ACTION:

Procedure SP(O) 4.3.2.1.1.a had been revised on March 9, 1982, to check 4KV supply breaker operation. The jumper was removed and breaker 1C4D was satisfactorily tested. Procedure SP(O) 4.3.2.1.1.a was satisfactorily completed at 1400 hours0.0162 days <br />0.389 hours <br />0.00231 weeks <br />5.327e-4 months <br />, March 28, 1982, and No. 1C SEC System was declared operable.

1 The sequence of events surrounding the installation of the jumper could not be determined. All SEC related wiring was checked against wiring diagrams, and no other problems were discovered.
