IDENTIFICATION OF OCCURRENCE:

No. 21~ Auxiliary Building Exhaust Fan - Inoperable.

This report was initiated by Incident Report 82-059.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 - Rx Power 100% - Unit Load 1150 MWe DESCRIPTION OF OCCURRENCE:

On March 26, 1982 an operator attempted to start No. 21 Auxiliary Building Exhaust Fan. The fan would not start and was declared inoperable. With No. 22 Auxiliary Building Exhaust Fan tagged out for maintenance, at 0258 hours0.00299 days <br />0.0717 hours <br />4.265873e-4 weeks <br />9.8169e-5 months <br /> Action Statement 3.7.7.c was entered.

This occurrence constituted operation in a degraded mode in accordance with Technical Specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The reason No. 21 Auxiliary Building Exhaust Fan would not start was the last time the fan circuit breaker had been racked in, the automatic trip shutter had not reset, preventing the breaker from closing.

ANALYSIS OF OCCURRENCE:

Technical Specification 3.7.7.c requires:

With only one exhaust fan operable, restore at least two exhaust fans to operable status within 7 days, or be in at least hot standby within the next 6 hours6.944444e-5 days <br />0.00167 hours <br />9.920635e-6 weeks <br />2.283e-6 months <br /> and in cold shutdown within the following 30 hours3.472222e-4 days <br />0.00833 hours <br />4.960317e-5 weeks <br />1.1415e-5 months <br />.

CORRECTIVE ACTION:

The operator reset the shutter, and the circuit breaker was tested satisfactorily. No. 21 Auxiliary Building Exhaust Fan was declared operable at 0315 hours0.00365 days <br />0.0875 hours <br />5.208333e-4 weeks <br />1.198575e-4 months <br />, March 26, 1982, and Action Statement 3.7.7.c was terminated.

The operation of station circuit breaker types has been recently incorporated into the operator training program.
