No. 12 Containment Fan Coil Unit - Inoperable.

This report was initiated by Incident Report 82-132.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 - Rx Power 100% - Unit Load 1150 MWe.

DESCRIPTION OF OCCURRENCE:

On May 26, 1982, during routine surveillance, the Control Room Operator discovered service water flow to No. 12 Containment Fan Coil Unit (CFCU) was less than required by the Technical Specifications. The unit was declared inoperable at 0935 hours0.0108 days <br />0.26 hours <br />0.00155 weeks <br />3.557675e-4 months <br />, and Action Statement 3.6.3.3.a was entered.

This occurrence constituted operation in a degraded mode in accordance with Technical Specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Flow Control Valve 12SW223 had stuck in the closed position, resulting in low service water flow to the CFCU.

ANALYSIS OF OCCURRENCE:

Technical Specification 3.6.2.3.a requires:

With one group of containment cooling fans inoperable and both containment spray systems operable, restore the inoperable group of cooling fans to operable status within 7 days, or be in at least hot standby within the next 6 hours6.944444e-5 days <br />0.00167 hours <br />9.920635e-6 weeks <br />2.283e-6 months <br /> and in cold shudown within the following 30 hours3.472222e-4 days <br />0.00833 hours <br />4.960317e-5 weeks <br />1.1415e-5 months <br />.

CORRECTIVE ACTION:

An investigation revealed that the valve tube bundle was extensively damaged by erosion and corrosion. A spare bundle was not available, and a spacer was installed in place of the damaged part. .The valve was exercised, and the CFCU satisfactorily tested. No. 12 CFCU was declared operable at 0020 hours2.314815e-4 days <br />0.00556 hours <br />3.306878e-5 weeks <br />7.61e-6 months <br />, May 28, 1982, and Action Statement 3.6.2.3.a was terminated.

The failure of service water SW223 valves is presently being investigated. A commitment was made in LER 82-029/03L to submit a Supplemental Report upon completion.
