' Report Number: 82-ll/03X-1 Report Date: 04-01-82 Occurrence Date: 02-15-82 Facility: Salem Generuting Station, Unit 1 Public Service Electric & Gas Company Hancocks Bridge, New Jersey 08038 IDENTIFICATION OF OCCURRENCE:

Fire Detection Instrumentation - Inoperable.

This report was initiated by Incident Report 82-034.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 6 - Rx Power 0% - Unit Load 0 MWe DESCRIPTION OF OCCURRENCE:

At 1100 hours0.0127 days <br />0.306 hours <br />0.00182 weeks <br />4.1855e-4 months <br />, February 15, 1982, the Control Operator noticed that the fire alarm for the 84' Elevation, Charging Pump Area had annun-ciated on the 1RP5 Fire Zone Alarm panel, but the coded audible alarm did not sound. The fire alarm was declared inoperable and Action Statement 3.3.3.6 was entered.

This occurrence constituted operation in a degraded mode in accordance with Technical Specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

A fire alarm relay was not seated firmly in it's socket.

ANALYSIS OF OCCURRENCE:

Technical Specification 3.3.3.6 requires: With the number of operable fire detection instruments less than required by Table 3.3-10:

Within one hour establish a fire watch patrol to inspect the zone (s) with the operable instrument (s) at least once per hour, and restore the inoperable instrument (s) to operable status within 14 days, or in lieu of any other report required by Specification 6.9.1, prepare and submit a special report to the commission pursuant to Specification 6.9.2 within the next 30 days.

CORRECTIVE ACTION:

An operator was immediately stationed at the 1RP5 panel to monitor.any incoming alarms.

The control circuit relay from the 24 hour2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> timer in the Miscellaneous Signal Cabinet was sticking. The audio / test switch on the timer was cycled, the relay cleared, and the coded audible alarm functioned properly. At 1420 hours0.0164 days <br />0.394 hours <br />0.00235 weeks <br />5.4031e-4 months <br />, February 15, 1982 Action Statement 3.3.3.6 was terminated.

On February 22, 1982 the audible alarm signal did not actuate during a fire alarm test. A1 fire alarm relay was not seated firmly in it's socket. The relay was firmly seated and the fire alarm audible signal was tested satisfactorily.
