' IDENTIFICATION OF OCCURRENCE:

No. 13 Steam Generator Wide Range Level - Inoperable.

This report was initiated by Incident Report 82-106.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 - Rx Power 89% - Unit Load 1024 MWe.

DESCRIPTION OF OCCURRENCE:

On May 1, 1982, during routine startup operation, the Control Room Operator discovered that No. 13 Steam Generator Wide Range Level indication had failed low. The channel was declared inoperable at 1055 hours0.0122 days <br />0.293 hours <br />0.00174 weeks <br />4.014275e-4 months <br />, and Limiting Condition for Operation 3.3.3.7.a Action 1 was entered.

This occurrence constituted operation in a degraded mode in accordance with Technical Specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

A piece of solid wire was found shorting the instrument signal-lead to-the conductor shield.

ANALYSIS OF OCCURRENCE:

Technical Specification 3.3.3.7.a Action 1 requires-With the number of operable accident monitoring channels less than the required number of channels, restore the inoperable channel (s) to an operable status within 7 days, or be in hot shutdown within the next 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br />.

CORRECTIVE ACTION:

The wire was removed and the channel was satisfactorily tested.

At 1545 hours0.0179 days <br />0.429 hours <br />0.00255 weeks <br />5.878725e-4 months <br />, No. 13 Steam Generator Wide Range Level was declared operable, and Limiting Condition for Operation 3.3.3.7.a Action 1 was terminated. The possibility of attempted sabotage was con-sidered, and immediate notification to the NRC was made in accor-dance with 10 CFR 50.72. Subsequent investigation showed that the wire was left in the cabinet after a recent design modifi-cation; the investigation results were summarized in a report by the SORC in meeting 82-47, May 5, 1982. This occurrence in no way endangered the general public or the environment.
