Abstract of Examinations An inservice examination (ISi) of selected Class 1, Class 2, and Class 3 components of Public Service Electric and Gas Company's (PSEG) Salem Generating Station, Unit 2, was perfonned by PSEG, Magnaflux Quality Services (MQS), The Virginia Corporation of Richmond (VCR) and Framatome Technologies (FTI) personnel during the Ninth Extended Shutdown Outage (June 8, 1995 -August 29, 1997) and Tenth Refueling Outage (April 3, 1999 - May 28, 1999). These examinations constituted the Ninth Extended Outage (2nd Period, 1st Outage) and Tenth Refueling Outage (2nc:1 Period, 2nd Outage) at Salem 2 during

. the Second 10-year Inspection Interval of operation.

The Extended Shutdown Outage #9 was not a refueling outage, and as such shall be reported under Refueling Outage# 10's NIS-1, Ninety (90) Day Report.

Exams were perfonned in accordance with the 1986 Edition of ASME Section XI and the Salem Unit 2 lnservice Inspection Program Long Tenn Plan.

PSI and ISi was perfonned utilizing visual (VT), magnetic particle (MT), liquid penetrant (PT), manual ultrasonic (UT), and radiography (RT) nondestructive examination techniques.

This report also contains augmented examinations required by the Salem Technical Specifications, Regulatory Guides, Circulars and Bulletins issued by the United States Nuclear Regulatory Commission and Public Service Electric and Gas.

The following abstract infonnation is broken down into two sections. The first section (11A) describes the Extended Outage #9 and the second section (11 B) describes Refueling Outage #10.

11A.2 ISi Section XI Visual Examinations on Valves and Bolting PSEG's ISi Group conducted five (5) Class 1 bolting exams and one (1) Class 1 valve internal exams.

The following was identified during the course of these examinations:

1. Valve 21SJ54 (WinlSI Sum# 164075) internal VT-3 examination revealed valve wedge cracks (Ref. AR# 951129066).

11A.3 ISi Section XI System Leakage Examinations PSEG's ISi Group conducted fifty three (53) System Pressure Tests during a System Functional or lnservice Test, on Nuclear Class 1, 2, and 3, in accordance with ASME Section XI.

A system Leakage exam was performed in accordance with ASME Section XI on all Class 1 Systems. The system leakage test was performed with the Reactor Coolant System in Mode 3 (normal operating temperature and pressure) with the insulation installed. No pressure boundary leakage was detected.

Work requests were initiated to correct minor mechanical leakage that was not corrected during the system walkdowns.

Class 2 and 3 System Pressure Tests were conducted on the following systems:

Hydraulic snubbers were tested using the Technical Specification 4.7.9(c), 10%

sample plan by Wyle Laboratories using the API in-place snubber test machine and the Wyle Laboratories model 150 snubber test machine as follows:

Mechanical snubbers were tested using the Technical Specification 4.7.9(c),

10% sample plan using the Wyle Laboratories model 150 snubber test machine as follows:

11A.11.3 HYDRAULIC AND MECHANICAL EXPANDED SNUBBER TEST SAMPLES Due to Hydraulic snubber test failures the following expanded snubber test samples were tested using the Wyle Laboratories model API snubber test machine.

One (1) expanded sample, totaling two (2) S/G 1,000 Kip Rexnords PSEG Engineering performed a stress evaluation on the failed snubber locations and concluded that the piping and components to which the snubbers were attached were not impaired due to in-operability of the snubbers.

One (1) Lisega small Snubber Nine (9) Mechanical medium Snubbers (PSA-1, 3, & 10) 100% of mechanical small snubbers were tested during the first sample, therefore three (3) mechanical small Snubbers (PSA-1/4 &

1/2) were tested.

Steam Generator Tube Eddy Current Inspection per Technical Specification 4.4.6.0 Over 143,000 inspections were performed to assess the overall condition of the Unit 2 steam generators. The examinations were performed by Framatome Technologies, Inc. (FTI) in accordance with their safety related QA program and as required by:

Salem Unit 2 Technical Specifications,Section XI, 1986 Edition, of ASME Boiler and Pressure Vessel Code, PSEG site specific data analysis guidelines.

A bobbin coil examination was performed on 100% of the in-service tubes in each steam generator. The bobbin coil exam was first attempted with a 0. 720" diameter probe for all tubes. If the 0. 720" probe was not successful in traversing the full length of the tube, the tube was reexamined using a 0. 700" or 0.680" diameter probe.

11A.12 Steam Generator Tube Eddy Current Inspection per Technical Specification 4.4.6.0 (Cont'd)

Motorized Rotating Pancake Coils (MRPC) were used to examine all hot leg tubesheet transitions and all hot leg tube support plates. This examination was expanded to include all of the cold leg tubesheet transitions and tube support locations in one steam generator (SIG 22). The MRPC inspections utilized a 3-coil probe that included a plus point coil, a 0.115" mid-range frequency coil and a 0.080" high frequency coil. Special interest MRPC examinations were performed on tube locations with a bobbin percent throughwall indication of 40% or greater or with I-code indications. Single coil plus point probes were used for row 2 LI-bends.

All examination techniques, except the low row L)-bend plus point MRPC examination, were qualified under the EPRI "PWR Steam Generator Examination Guidelines," document NP-6201, Appendix H at the time of the examination. Since the examination, a plus point technique for examination of the U-bend regions was qualified and approved for Appendix H. The approved variables were similar to those used for examination of U-bend regions at Salem Unit 2.

A number of cold leg support plate intersections were identified as having volumetric indications. Both the bobbin coil and MRPC examination reported this damage mechanism. Those tubes remaining in-service are below the required plugging limit as sized by the bobbin coil examination. This is an approved EPRI Appendix H qualified sizing technique for cold leg thinning.

Based on both the bobbin and MRPC exams, 220 tubes were plugged. 41 tubes in 21 SIG 21, 52 tubes in 22 SIG, 22 tubes in 23 SIG and 105 tubes in 24 SIG. The indications were classified into seven categories:

Outer diameter intergranular attack/stress corrosion cracking (ODSCC) at tube support plates, ODSCC at the hot leg top-of-tubesheet sludge pile, Primary side stress corrosion cracking (PWSCC) at the hot leg tubesheet expansion region, Corrosion assisted wastage at cold leg tube support plates, Wear at the anti-vibration bars, Possible loose part damage, Indications at tube support plates other than ODSCC (plugged as a preventative measure).

All tube exceeding the technical specification criteria or having crack like indications were plugged. The categorization of each Steam Generator is listed below and takes into consideration both bobbin coil and MRPC inspection results:

Valve 24SJ138 to pipe weld 2-SJ-1247-7 (WinlSI Sum# 186850) was found with a PT indication (Ref. AR# 990414184). After surface buffing to determine indication relevancy, the indication was found to be non recordable.

There were no other indications identified.

11 B.2 ISi Section XI Visual examinations on valves and bolting PSEG's ISi Group conducted ten (10) Class 1 bolting and no Class 1 valve internal exams.

The following was identified during the course of these examinations:

Valve 2RH1's bolting (WinlSI Sum# 257850) was VT-1 examined in-place. The bolting was found with boron residue. The bolt nearest the leak was removed and was satisfactorily VT-3 examined. The boron residue was removed and the bolt torque was re-verified as required (Ref. AR# 990414183)

The following was identified during the course of these examinations:

1. Component support 2-FWH-23-17 (WinlSI Sum# 330275) was VT-3 examined, and was found with the spring can setting out of tolerance (Ref.

AR# 990414195). The component supports on either side of2-FWH-23-17 and an additional sample was satisfactorily VT-3 examined. Component support 2-FWH-23-17 was satisfactorily re-adjusted by Mechanical Maintenance in accordance with W.0.# 990510253.

There were no other unacceptable conditions identified.

11 B.4 ISi Section XI System Leakage Examinations PSEG's ISi Group conducted four (4) System Pressure Tests during a System Functional or lnservice Test, on Nuclear Class 1, 2, and 3, in accordance with ASME Section XI.

A system Leakage exam was performed in accordance with ASME Section XI on all Class 1 Systems. The system leakage test was performed with the Reactor Coolant System in Mode 3 (normal operating temperature and pressure) with the insulation installed. No pressure boundary leakage was detected.

Work requests were initiated to correct minor mechanical leakage that was not corrected during the system walkdowns.

118.15 Steam Generator Tube Eddy Current Inspection per Technical Specification 4.4.6.0 During the Salem Unit 2 10 111 Refueling Outage in April of 1999 Framatome Technologies Incorporated (FTI) conducted Eddy Current examinations on 21 through 24 steam generators. The inspections were performed after chemical cleaning. 22 and 23 were the first two steam generators to be inspected followed by 21 and 24. In general the bobbin coil inspections were performed from the cold leg and rotating coil (top of tubesheet and dented tube support plate intersections) inspections performed from the hot leg. The exception to this was SG 21 where the bobbin coil inspectio" was performed from the hot leg due to ongoing plug repair efforts in 23 SG HL.

The scope of the inspection was delineated in the 2R 10 SIG Tubing Degradation Assessment. This document identified the active and potential degradation mechanisms affecting the tubing in the Salem Unit 2 Steam Generators, identified the inspection scope and techniques to be used, and provided structural limits for each damage mechanism for assessing tube integrity requirements. The: base eddy current examination scope met Salem Unit 2 Technical Specification requirements. A summary of the base scope follows:

Full-length bobbin coil inspection of 100% of the in-service tubes in each steam generator.

Rotating Coil (+ Point) exam of a 20% sample of the row 2 short radius U-bends (07C-07H) in each steam generator.

Rotating Coil(+ Point) exam of 100% of the hot leg WEXTEX tubesheet transition regions in each steam generator.

Rotating Coil(+ Point) exam of 100% of the >2 volt dented tube support plate intersections, based on 2R9 bobbin coil data, in the defined "critical area" for each steam generator.

11 B.15 Steam Generator Tube Eddy Current Inspection per Technical Specification 4.4.6.0 (Cont'd) 11B.15.4 PWSCC in Hot Leg Tubesheet and Tube Support Regions Axial and Circumferential PWSCC was identified in the hot leg tubesheet during 2R 10. All of the tubes with indications were subject to an historical review for detection, sizing, and growth rates for condition monitoring. No tube required stabilization during 2R10.

Steam generator 21, tube R15 C13 had the only axial PWSCC indication at a tube support elevation. This indication was located at 02H in a 2.41-volt dent.

The bobbin coil examination did not identify this tube support as distorted.

11B.15.5 AVBWear Wear was identified in the U-bend region of all steam generators. This mechanism has been attributed to vibration of the tube against the anti-vibration bars.

Recent eddy current examinations at other sites with Westinghouse Series 51 steam generators brought to light the need for an additional AVB standard with deeper flaws than the traditional 20% and 40% provided in Salem's inline guidetube standards. The deeper insitu flaws are of a voltage that exceeds the upper limit of the calibration curve, thereby raising doubt about the accuracy of indications that are close to the repairable limit of 40%. This standard was manufactured with AVB defects ranging from 30% to 80% through wall. The indications that exceeded the curve in S/G's 22 and 23 were run again with this standard, and a five-point curve was established using the-as-built dimensions. Based on this second run with the deeper standard flaws, the through wall depths of the original run were substantiated, and no addition tubes were plugged.

118.15 Steam Generator Tube Eddy Current Inspection per Technical Specification 4.4.6.0 (Cont'd) 118.15.6 Low Row U-bend Indications During the 20% +Point examination of the row 2 LI-bends in steam generator 23, an inside diameter single circumferential indication was identified in the hot leg tangent of tube R2 C9. This forced an expansion to 100% of the row 2 U-bends, and a 20% sample of the row 3 u-bends in S/G 23. During the expansion three more row 2 tubes were identified as having similar SCI indications.

A review was performed on the 2R9-+point U-bend data for these tubes.

These indications were present in the 2R9 data, and do not exhibit any noticeable change in signal phase or amplitude from last outage.

118.15.7 Manufacturer's Burnish Marks MBM's were identified with the bobbin coil examination. All freespan indications indicative of an MBM type signal were compared to the 1983 data for historical comparison and to identify significant change in the signals between the two examinations. Any change based on the parameters of the freespan flow chart were identified as an "I" code and further examined with a

+Point probe. Indications not exhibiting change based on the parameters of the flow chart were left in the database with the code "MBH". None of the "MBI" indications were confirmed as crack-like when examined with the +Point probe.

118.15 Steam Generator Tube Eddy Current Inspection per Technical Specification 4.4.6.0 (Cont'd) 118.15.8 Freespan Differential Signals As compared to the M8M signal, freespan differential signals exhibit sharper signal characteristics, which could be indicative of damage mechanisms such as IGA/SCC or pitting. These signals were also compared to the 1983-outage data, as well as the 1996 data, to detect change in phase and voltage. If no change was noted the indication was left in the database as "FSH". Any "FSI" indication was put on the special interest program for +Point examination. All of these indications were NDD'd by +Point with the exception of one tube.

R18C9 in S/G 21 had eleven (11) bobbin indications that were not evident in the 1996 data. The +Point probe identified 18 axial indications along the same axial plane between the hot leg tubesheet and the first support. The mid frequency identifies what appears to be two axial scratches that run between this span, and these indications occur along the length of one of these scratches.

All of the tubes from the same heat lot as tube R 18 C9 were re-evaluated by the lead analyst in steam generator 21 from TSH to 01 H on the bobbin coil data looking for similar indications, and none were noted.

11 B.15 Steam Generator Tube Eddy Current Inspection per Technical Specification 4.4.6.0 (Cont'd) 11B.15.9 Suspect Ligament Cracking All bobbin data was analyzed for possible support indications with CDS. The indications identified during this analysis were RPC (+Point) examined to determine if a change had occurred on the previously identified tube support elevations, and as a baseline on the new indications identified during 2R10. No suspect ligament cracks (SLC's) were identified as having a >145 degree gap that would exceed to threshold for repair of these types of indications. New ligament indications were identified by the bobbin method during this examination, and this is attributable to the chemical cleaning of the steam generators. Comparison of the +point data between 2R10 and 2R9 confirms this, as these indications were evident during that examination as well.

In 1998, a study was performed on the 2R9 data to identify possible breached supports. One of the action items from this review was to run the bobbin data at 10 kHz for comparison to the historical low frequency, which was changed in the 2R9 outage to 20kHz. Upon direct comparison of historical indications to the 10 kHz data this outage, it was determined that the change in frequency from 10 kHz to 20 kHz had no influence on the signal formations from outage to outage.

From the 1998 review, one tube in S/G 23, tube R22 C75, was identified as having a missing section of tube support plate that measured 38° by eddy current. This indication was visually verified in that the tube support plate is mis-drilled, and the tube is physically breached into the flow hole.

Also from the 1998 review, one tube in SIG 24, tube R46 C41, was identified as having multiple indications indicating a possible breach. This tube was visually verified to be a single crack in the support plate immediately adjacent to the patch plate weld.

118.15.11 Loose Parts The eddy current data was manually analyzed for loose parts two tubes around the entire periphery and down the divider plate. There was one possible loose part confirmed with the +point coil during this examination in SIG 23, tube R28 C76.

After chemical cleaning a visual examination was performed in the areas of the steam: generators where loose parts were visually confirmed during 2R9. Two of the objects identified have been retrieved, and the balance was relocated during the chemical cleaning and sludge lancing processes.

There.were two tubes identified during the 2R7 that exhibited signs of loose part wear above the hot leg tubesheet in SIG 24. The part was removed and these tubes subsequently left in service. During 2R10, these indications were sized with the techniques available for the +Point coil using the AVB wear standard, which is indicative of the type of flaws produced by loose parts wearing into the outside diameter of steam generator tubing.
