No. 11 Safety Injection Pump - Inoperable.

This report was initiated by Incident Reports82-112 and 82-113.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 - Rx Power 65% ~ Unit Load 660 MWe.

DESCRIPTION OF OCCURRENCE:

On May 2, 1982 while testing Valve 11SW122 following maintenance, a No. lA Safeguards Equipment Control (SEC) Cabinet actuation occurred. No. lA Diesel Generator started and all safety related loads, with the exception of No. 11 Safety Injection Pump, were energized. At 1358 hours0.0157 days <br />0.377 hours <br />0.00225 weeks <br />5.16719e-4 months <br />, No. 11 Safety Injection (SI) Pump was declared inoperable,'and Action Statement 3.5.2.a was entered.

No apparent reason for the No. lA SEC actuation could be found; the SEC signal was reset at 1427 hours0.0165 days <br />0.396 hours <br />0.00236 weeks <br />5.429735e-4 months <br />, and all safety loads were returned to normal.

This occurrence constituted operation in a degraded mode in accordance with Technical Specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The supply breaker to No. 11 Safety Injection Pump locked out on the anti-pump feature due to a spurious SEC Cabinet actuation; the SEC signal was due to a failed card file assembly in con-junction with noise from the Valve llSW122 close solenoid.

ANALYSIS OF OCCURRENCE:

Technical Specification 3.5.2 requires:

That two independent ECCS subsystems shall be operable with each subsystem comprised of one operable centrifugal charging pump, one operable safety injection pump, one operable residual heat removal heat exchanger, one operable residual heat removal pump, and an operable flow path capable of taking suction from the Refueling Water Storage Tank.

With one ECCS subsystem inoperable, restore the inoperable subsystem to operable status within 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br />, or be in hot shutdown within the next 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br />.

CORRECTIVE ACTION An investigation of the failure of No, 11 SI Pump showed the supply breaker closing springs were charged, and the breaker was locked out on the anti-pump feature. The breaker and motor windings were satisfactorily tested, and the breaker lock out was attributed to

the spurious nature of the SEC actuation. At 2105 hours0.0244 days <br />0.585 hours <br />0.00348 weeks <br />8.009525e-4 months <br />, May 2, 1982, No. 11 SI Pump was declared operable, and Action Statement 3.4.2.a was terminated.

Further investigation showed that the spurious SEC signal resulted

from a failed card file assembly in conjunction with noise gener-ated by Valve 11SW122. The SEC was properly repaired and satis-factorily tested. Design Change Request lET-1349 has been issued to install noise suppression diodes on the valve operator solenoids.

A Supplemental Report will be submitted upon completion.
