Penetration Fire Barriers - Non-Functional.

This report was initiated by Incident Report 82-177.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 - Rx Power 100% - Unit Load 1100 MWe.

DESCRIPTION OF OCCURRENCE:

On July 12, 1982, while performing Maintenance Procedure M3Y-2, two floor penetrations in the Control Room were found to be non-functional and not documented. Penetrations 1411A1000lF and 2F were involved; the fixtures at the openings were loose, and the openings not sealed. Technical Specification Action Statement 3.7.11 was already in effect dating back to 0001 hours1.157407e-5 days <br />2.777778e-4 hours <br />1.653439e-6 weeks <br />3.805e-7 months <br />, October 29, 1981, due to other open penetrations.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The penetrations were apparently left open at the time of installation. Sealing the openings may have been overlooked because the terminal fixtures obscured the penetrations.

The problem was overlooked in subsequent fire barrier surveillances, probably for the same reason. Final discovery was related to the recent implementation of a more detailed fire barrier surveillance i procedure.

ANALYSIS OF OCCURRENCE:

The functional integrity of penetration fire barriers ensures that fires will be confined or adequately retarded from spreading to adjacent portions of the facility. This design feature minimizes the possibility of a single fire affecting several areas containing redundant safety related equipment. i unsealed opening does not in itself represent significant risk to the health and safety of the public.

ANALYSIS OF OCCURRENCE: (continued)

As such, the event constituted operation in a degraded mode permitted by a limiting condition for operation and is reportable in accordance with Technical Specification 6.9.1.9.b.

Action Statement 3.7.11 requires:

With one or more of the above required penetration fire barriers non-functional, establish a continuous fire watch on at least one side of the affected penetration within 1 hour1.157407e-5 days <br />2.777778e-4 hours <br />1.653439e-6 weeks <br />3.805e-7 months <br />.

CORRECTIVE ACTION:

The Control Room is already manned on a 24 hour2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> basis, fulfilling the requirements of the action statement. The penetrations were properly sealed July 12, 1982, and were added to Maintenance Procedure M3Y-2 surveillance. The procedure contains provisions to identify and include additional non-functional barriers.
