Region III has recently received anonymous allegations concerning activities

. associated with Metalweld Inc., a safety related coating contractor at the Perry Nuclear Power plant. .These allegations'were previously discussed with you during an October 25, 1985, telephone call between you and Messrs.

R. C. Knop and C. H. Weil of our staff. Enclosure 1 is a description. of the allegations received. -

Due to the' question of coating adequacy raised by the allegations provided in enclosure 1, Region III expects that Cleveland Eldctric Illumina~ ting Company (CEI) will analyze the concerns; perform necessary audits, surveillances, or

, appropriate investigations in order to determine the validity and significance of the alleged conditions; and take appropriate actions as necessary in order to address both the specific concerns and root causes, if. applicable. .

We further request that the results of your review and disposition of these matters be submitted to Region III after completion. Documentation supporting your results should be maintained onsite for any further inspections by Region III. You should also be aware that the technical aspects of these allegations must be adequately resolved prior to fuel load.

The enclosures to this letter are being withheld from public disclosure in accordance with Title 10, Code of Federal Regulations, Part 2.790(a)(b) and (7).

ENCLOSURE I ALLEGATIONS

1. Approximately 45 individuals (see enclosure 2 for a list of the individuals), including management, technical, quality control and

- craft personnel, were identified as users and/or sellers of controlled )

substances (primarily cocaine and marijuana). '

. Marijuana was used daily within the plant protected area. The l alleger mentioned one occasion when a Metalweld QC inspector inside the protected area stated, " Boy I'm really high... I just smoked some great reefer."

. According to the alleger, the results of cocaine abuse were  !

evident in the personal life of a metalweld engineer.

. An alleger expressed his view that a prompt response to the allegations including urinalysis of named individuals would surely substantiate the allegations.

. An alleger stated that firearms are being sold in the Perry parking lot on many occasions. .

2. An alleger stated that paint was peeling on the 689 foot elevation of the containment (the interior surface of the dome) in the vicinity of the 1 containment spray ring header second from the outside at the 270 azimuthal coordinate. The alleger attributed this condition to inadequate surface preparation (removal of dirt and scale) and excessive containment liner temperatures.

3. An alleger stated that cracks in the coating applied on welds on a crane box beam in containment were due to cracks in the welds.
4. Paint was peeling on the refueling floor bridge crane observation platform.

A " needle gun" (chipping tool) was used to remove the coating without disturbing the coated surface. The surface was found to be badly rusted ,

indicating a disregard for surface preparation requirements. '

5. Painters have used tungsten carbide-tipped drill bits on their buffing and grinding tools to prepare weld surfaces for coating application thereby removing excessive weld metal from coated items such as snubber brackets, pipe supports, pipes, etc. Metalweld had been instructed by CEI QC to discontinue this practice on several occasions. While the drill bits are no longer maintained in or issued from the Metalweld tool crib, painters are in possession of them and continue to use them. An l alleger stated that he had. observed this practice as late as the morning of October 25, 1985.
