Gentlemen:

This refers to the routine safety inspection conducted by Messrs. J. A. Grobe,

K. A. Connaughton and J. W. McCormick-Barger of this office on April 9 through

May 12, 1986, of activities at Perry Nuclear Pcwer Plant, Unit 1, authorized

by NRC Facility Operating License No. NPF-45 and to the discussion of our

findings with Mr. C. M. Shuster at the conclusion of the inspection.

The enclosed copy of our inspection rcport identifies areas examined during

the inspection. Within these areas, the inspection consisted of a selective

examination of procedures and representative records, observations, and

interviews with personnel.

During this inspection, certain of your activities appeared to be in violation

with hRC requirements, as specified in the enclosed Notice. A written

response is required.

Pursuant to NRC's Enforcement Policy, we have carefully evaluated the

violations identified to determine whether or not escalated enforcement action

was warranted. We have concluded that the safety significance of the

violations was minimal, but note that this was due largely if not solely to

plant conditions at the time of each occurrence. For this reason escalated

enforcerrent action will not be taken at this time. Notwithstanding, and as we

have emphasized to you during the course of our routine meetings in the past,

this trend merits prompt and aggressive senior licensee management attention to

preclude similar violations in the future. Your response to the enclosed

notice, which underscores our concerns, should specifically address ycur

program for identification of rcot cause(s) of violations and those steps you

plan or have already taken to assure prompt and ccmprehensive corrective

action is taken to preclude occurrences involving similar personnel errors.

Your response to the enclosed Notice should include consideration of the

possible contribution to these occurrences of the ambitious scheduling of work

activities requiring the cognizance and support of operating personnel.

this letter, the enclosures, and your response to this letter will be placed

in the NRC Public Document Room.

The responses directed by this letter and the accompanying Notice are not

subject to the clearance procedures of the Office of Management and Budget as

required by the Paperwork Reduction Act of 1980, PL 96-511.

We will gladly discuss any questions you have concerning this inspection.
