This refers to an Enforcement Conference conducted by Dr. Carl J. Paperiello,
l
Deputy Regional Administrator, and other members of the NRC staff with you and
l
l
members of your staff on June 7, 1989, at the Region III Office in Glen Ellyn,
j
l
Illinois, to discuss work performed in the vicinity of an unusually high,
!
localized radiation field on March 24, 1989, and the potential for an
j
overexposure as a result of inappropriate radiological controls over this
i
work.
Details of this matter are documented in NRC Inspection Report
1
No. 50-440/89016(DRSS).
i
After review of the information presented at the Enforcement Conference by
you and your staff, we have determined that a substantial potential for an
overexposure did not exist in this case because the high radiation fields were
very localized and the nature of the work made it unlikely that the workers
i
spent a significant amount of time in those fields.
This determination was
l
made only after considerable deliberation, which reflected the seriousness
l
which we believe this matter represented.
We are particularly ccncerned that
appropriate radiation surveys were not made, that a week-old survey was used
to allow the initial entry into the work area, that except for your dose
reconstruction, your followup of this incident lacked aggressiveness and depth,
and that we were not informed of this significant breakdown in your radiation
protection program.
We expect that the lessons-learned from this incident will
be incorporated in your training program in an attempt to preclude future
l
similar problems.
Recurrences of these problems may result in escalated
enforcement in accordance with the NRC Enforcement Policy.
l
During this inspection (Inspection Report No. 50-440/89016), certain of your
activities appeared to be in violation of NRC requirements, as described in
the enclosed Notice,

In accordance with 10 CFR 2.790 of the Commission's regulations, a copy of
