Gentlemen:
This refers to the followup inspection conducted by Mr. P. L. Hiland of
this office on August 7 through 23, 1989, of your response to the
Diagnostic Evaluation Team report and to the discussion of our findings with
Mr. M. W. Gmyrek and others of your staff at the conclusion of the inspection.
The enclosed copy of our inspection report identifies areas examined during
the inspection. Within these areas, the inspection consisted of a selective
examination of procedures and representative records, observations, and
interviews with personnel.
During this inspection, certain of your activities appeared to be in violation
with NRC requirements, as specified in the enclosed Notice. With regard to
items 1 and 3 a written response is required. With regard to item 2, your
response to the DET report provided adequate corrective action as discussed
in paragraph 2.g of the enclosed report and a further response is not required.
In accordance with 10 CFR 2.790 of the Commission's regulations, a copy of
this letter and the enclosed inspection report will be placed in the NRC
Public Document Room.
In addition to the response required by the enclosed Notice, an additional
response is requested regarding the item discussed in paragraph 2.t of the
enclosed report.
The responses directed by this letter and the accompanying Notice are not
subject to the clearance procedures of the Office of Management and Budget
as required by the Paperwork Reduction Act of 1980, PL 96-511.
