No. 12 Waste Monitor Tank - Liquid Release Sample Discarded.

This report was initiated by Incident Report 82-179.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 - Rx Power 100% - Unit Load 1110 MWe.

DESCRIPTION OF OCCURRENCE:

On July 10, 1982, in preparation for liquid release lLL 119-82 No. 12 Waste Monitor Tank contents were sampled. The isotopic analysis for principal gamma emitters was satisfactory, and the release was conducted. The tank sample was subsequently held for compositing into weekly, monthly and quarterly samples. Before the composite samples were assembled, however, contents of the bottle containing the Release lLL 119-82 sample were discarded.

Proper compositing of the samples could not be performed, in violation of Environmental Technical Specification (ETS) Limiting Condition for Operation 2.3.2. The liquid release was routine in nature; no unusual radioactive materials or concentrations were present in the monitor tank.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

A technician looking for an empty sample container inadvertently poured out the monitor tank sample. The technician had not realized that retention of the sample was necessary for compositing future samples.

ANALYSIS OF OCCURRENCE:

The sampling and monitoring requirements in ETS Section 2.3.2 provide assurance that radioactive materials in liquid wastes .

are properly controlled and monitored. The design objectives of controlling releases are to limit the whole body or organ dose to an individual in an unrestricted area to 5 mrem /yr, and to limit annually the total quantity of radioactive materials discharged in liquid waste to 5 C1 (excluding tritium and dissolved gases).

The levels of longer lived isotopes involved in Release ILL 119-82 were extremely low, as extrapolated from the isotopic analysis and comparison with similar samples. The occurrence, therefore, did not involve any adverse impact on the environment.

In the event a limiting condition for operation is exceeded 7 Section 5.4 of the ETS requires:

1) Remedial action as permitted by the ETS shall be taken until the condition can be met.
2) Exceeding a limiting condition for operation shall be investigated by the Station Operations Review Committee (SORC) .
3) A report of the occurrence shall be prepared and submitted as specified by Section 5.6.2.

CORRECTIVE ACTION:

No remedial action was deemed necessary in view of the routine nature of the release. The technician involved in the event was counseled by the Chemistry Supervisor concerning ETS Section 2.3.2 and the compositing procedures. Samples being held for compositing are already placed in a designated area; the technician was not aware of the storage program, however. The sample of similar Release lLL 121-82, which occurred July 13, 1982, will be double composited to simulate inclusion of the lost sample and to meet the limiting condition for operation. Finally, this report ,

satisfies requirements of Section 5.6.2, and review of the report by the SORC completes compliance with Section 5.4.
