+e Trojan Nuclear Plant Mr. D. F. Kirsch Docket 50-344 December 20, 1985 License NPF-1 Attachment Page 1 of 1 10 CFR 2.201 RESPONSE TO NOTICE OF VIOLATION Violation As a result of the inspection conducted during the period of October 3 -

November 16, 1985, and in accordance with the NRC Enforcement Policy, 10 CFR Part 2, Appendix C, the following violation was identified.

10 CFR 50.59, paragraph (b) requires in part that changes to the facility as described in the safety analysis report include a written safety evaluation which provides the bases for the determination that the change does not involve an unreviewed safety question.

Contrary to the above, on October 3, 1985, two helium gas bottles were observed strapped to the Seismic Category I supply and return lines of the "B" train containment hydrogen analyzer without a supporting safety evaluation having been performed.

This is a Severity Level IV violation (Supplement I).

Response

The helium bottles had been delivered to the 75-ft elevation of the Auxiliary Building for the performance of Technical Specification-required testing. The bottles were strapped to the hydrogen analyzer supply and return lines to prevent undesired movement in the interest of Plant and personnel safety. The potential adverse affect on safety-related equipment created by attaching the bottles to a Seismic Category I line was not recognized.

Immediate corrective action was taken to remove the helium bottles from the Seismic Category I lines. Additionally, inspections of the Plant were conducted to ersure no other similar situations existed. In order to prevent a reoccurrence of this situation, all managers and supervisors have been made aware of this event and have been directed to take action to further instill in their employees what constitutes a change to the Trojan facility and the need to follow approved procedures when making such a change.
