Missed Surveillance - Shutdown Margin Calculation.

This report was initiated by Incident Report 82-369.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 5 - Rx Power 0% - Unit Load 0 MWe.

DESCRIPTION OF OCCURRENCE:

On October 18, 1982, during routine shutdown operations, it was discovered that the shutdown margin calculation had not been performed within the interval required by Technical Specification Surveillance Requirement 3.1.1.2. The calculation had been satisfactorily completed at 0915 hours0.0106 days <br />0.254 hours <br />0.00151 weeks <br />3.481575e-4 months <br />, October 17, 1982, and was due, including the 25% allowance, at 1515 hours0.0175 days <br />0.421 hours <br />0.0025 weeks <br />5.764575e-4 months <br />, October 18, 1982.

With the failure to determine the shutdown margin, Action Statement 3.1.1.2 applied. The surveillance was satisfactorily performed at 2100 hours0.0243 days <br />0.583 hours <br />0.00347 weeks <br />7.9905e-4 months <br />, October 18, 1982; the shutdown margin was greater than 1.0% Ak/k.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The shutdown margin calculation was not performed due to personnel oversight. A review of the administrative and procedural controls involved in the scheduling and performance of periodic surveillances revealed no major problems. No recent similar occurrences were noted, and the event was assumed to be of an isolated nature.

ANALYSIS OF OCCURRENCE:

The requirement for shutdown margin in Mode 5 insures that the reactivity transients associated with postulated accident conditions are controllable within acceptable limits and that the reactor will be maintained sufficiently subcritical to preclude inadvertent criticality in the shutdown condition. Since the shutdown margin was subsequently determined to be within specification, the incident involved no risk to the health and safety of the public. The event constituted operation in a degraded mode permitted by a limiting condition for operation, and is reportable in accordance with Technical Specification 6.9.1.9.b.

Action Statement 3.1.1.2 requires:

With the shutdown margin <l.0% Ak/k, immediately initiate and continue boration at > 10 GPM of 20,100 ppm boric acid solution or equivalent until the required shut-down margin is restored.

CORRECTIVE ACTION:

As noted, the shutdown margin was >l% Ak/k, in compliance with the action statement. With satisfactory completion of the surveillance, the action statement no longer applied. Personnel involved with the incident were counseled concerning 'the Technical Specification surveillance requirements for shutdown margin. In view of the nature of the occurrence, no further action was deemed necessary.
