Dear Mr. Martin:

Deficiencies. Identified in Licenseo Event Report (LER) 88-19, Revision 1, " Surveillance Required by Technical Specifications Not Performed Following Containment Hydronen Vent System Absorber Replacement" Your letter of February 8, 1989 identified deficiencies with Revision 1 to LER 88-19 issued on November 11, 1988. Revision 2 was subsequently issued on March 31, 1989 and addresses the concerns raised in your letter. The LER now includes a discussion on: j i

1. Failure of craftsman to adequately recognize that the system (CS-9B) f being worked on was not under clearance, q l
2. An apparent violation of Plant Safety Procedure 3-30, Revision 18 j regarding proper description of the wor'k to be performed, the equipment j to be worked on, and the area in which the work would be performed. ]

3. The apparent inconsistencies between two test results on the "A" Train j filter of the hydrogen vent system. l
4. Field observation of the work by the Maintenance Work Group Supervisor in accordance with Administrative Order 3-9, " Maintenance Requests".
5. Independent reviews of the completed maintenance request by the Shift Supervisor and the Maintenance Work Group Supervisor.
6. Timeliness of LER revisions.
