Containment / Plant Vent Monitors - Improper Alignment.

This report was initiated by Incident Report 82-060.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 5 - Rx Power 0% - Unit Load 0 MWe.

DESCRIPTION OF OCCURRENCE:

A gaseous release from the Waste Gas System was initiated at 2250 hours0.026 days <br />0.625 hours <br />0.00372 weeks <br />8.56125e-4 months <br />. March 26, 1982. The Control Room Operator failed to align the Containment / Plant Vent Monitors 1RllA, 1R12A and 1R12B to the plant vent as required by Operating Instruction O.I.II-12.3.3.

This occurrence violated Limiting Condition for Operation (LCO) 2.3.4.c of the Environmental Technical Specifications (ETS), and therefore required a Nonroutine Report in accordance with ETS 5.6.2.1.a DESIGNATION OP APPARENT CAUSE OF OCCURRENCE:

The failure of the operator to properly perform the operating instruction resulted in the incorrect vent monitor alignment.

ANALYSIS OF OCCURRENCE:

Environmental Technical Specification 2.3.4.c requires:

During the release of gaseous wastes from the primary system waste gas holdup system, the gross activity monitor, the iodine collection device, and the particulate collection device shall be operating.

CORRECTIVE ACTION:

LCO 2.3.4.c was met at 0048 hours5.555556e-4 days <br />0.0133 hours <br />7.936508e-5 weeks <br />1.8264e-5 months <br />, March 27, 1982, with the termination of the gaseous release.

It was determined that.all other portions of the operating instruction were properly followed. The Waste Gas Effluent Monitor 1R14 and the Plant Vent Monitor 1R16, were operable and in service during the release. ETS release rate limits were not exceeded, and there was no adverse impact on the environment.

The operator involved was counseled by the Senior Shift Supervisor concerning LCO 2.3.4.c and the importance of adherence to approved procedures.
