Region III has received an allegation concerning the repair of alleged improper insulation material used to seal around small bore piping inside penetration sleeves and the repair of the alleged improper caulking material used to seal.

We further request that the results of your review and disposition of these concerns be submitted to Region III by April 30, 1986. I ALLEGATION

SUMMARY

The first cor.cern revolves.around what were called " mechanical penetrations" in either the containment or drywell where " insulation material" between the piping and a sleeve was alleged to be faulty. It was reported that the sleeves were approximately 3" and the piping 2-7/8" in diameter. The material had to be dug out and this was alleged to be very difficult because of the small annular space that was filled with insulation material. The alleger indicated that approximately 2000 of these penetrations had to be fixed and did not believe that they all were fixed.

The second concern revolves around small bore piping approximately 2" in diameter which are in clusters of 50 to 60 pipes at a penetration through a wall. The wrong type of caulking material was alleged to have been used to seal around the piping cluster. Again the material was reported to have had to be dug out and "it was like digging concrete with a knife". It was speculated that this piping may have been the control rod drive system piping. The alleger also did not think this work had been completed, but was aware that 10% of the work had been done.
