Gentlemen:

This refers to the special safety inspection conducted by Messrs. G. F. O'Dwyer,

J. A. Holmes, J. A. Grobe and C. F. Gill of this office from June 23 through

August 21, 1986 of activities at Perry Nuclear Power Plant authorized by NRC

Construction Permit No. CPPR-148 and to the discussion of our findings with '

Mr. M. D. Lyster and other members of your staff at the conclusion of the

inspection.

L The enclosed copy of our inspection report identifies areas examined during

.the inspection. Within these areas,.the inspection consisted of a selective

. examination'of procedures and representative records, observations, and

interviews with personnel. ,

During this inspection, certain of your activities appeared to be in violation

of NRC requirements, as specified in the enclosed Notice. 'A written response

! is required.

,

' In addition'to the apparent violations discussed above, we are concerned with

4 what appears to have been an absence of an aggressive, timely and effective

4

-response to the off-normal indications of fire in'the charcoal adsorber beds

as discussed in paragraph 6.e of the enclosed report. Please include with

your written. response to the Notice of Violations, what steps you have or will

be taking to ensure a more effective response to indications of off-normal

t conditions. r

! In accordance with 10 CFR 2.790 of the Commission's regulations, a copy of

l this letter, the enclosures, and your response to this letter will be placed

in the NRC Public Document Room.
