Caune of Event The cause of the occurrence was a desien de f i c i e r .v . The oririnal des ~n s h ou ld have included s of t -s ea t e d check valves for the ADS air sunnly.

Corrective Action

.' n . Powell Co. h a r d -:.; e a t e d check valves have 'n c e n replaced with ;upro Co. soft-seated chech valves. Acceptable test results vere attain?d and the Units returned to service.
