SUIUECT: Waterford 3 SES Iluman Factors Control Room Review on April 13, 14, and 15, 1982

Dear Mr. Tedesco:

It has become apparent from the Waterford 3 control room reviews that have been conducted to date that many features have been implemented in the control board design that are consistent with the latest human engineering guidelines. Since TMI a number of changes have been made to enhance operations and provide for a more human engineered control board. A lint of these changes is included as Attachment A.

On April 15, 1981, we submitted our " Preliminary Control Room Assess-ment." This report documented our review performed at the end of 1980.

Since the submission of the preliminary report and the changes imple-mented in Attachment A, LP&L has initiated an ongoing program to ex-amine the features of the control panels by experts in human factors as applicable to the operator / control room interfaces. This program has identified several areas of concern. In perspective, these con-cerns derived from strictly a human engineering viewpoint and must be evaluated to determine if any corrective action is warranted. If it is determined that corrective action is required, then a further evalu-ation will be performed to decide the optimum corrective action con-sidering a?A factors and constraints relateu to our project.

Part of the ongoing review involved the formation of an !!ED committee, comprised of specialist in commercial nuclear power plant operation, engineering, training, and human factors, and that this committee is meeting periodically to review control room status and evaluate HEDs.

To further enhance our ability to perform continuing control room design reviews, LP&L has constructed a half-scale mockup of the control room panels.

As an adjacent effort, a review of plant systems using failure modes and effects analysis, common cause failure analysis, and Event-Tree / Fault Tree Analyses is ongoin9-

ATTACHMENT B WATERFORD 3 SES CONTROL ROOM STATUS Construction is essentially complete in the Control Room. Many of the systems, however, are not turned over to LP&L, therefore a number of systems are not operating (ie. IIVAC).

o The pre-requisite test on the ESFS is approximately 35% complete. The plant protection system is in the process of being tested. The above systems are expected to be completed by August 1982.

o Normal and emergency lighting systems are working but not tested. The release package is scheduled for issue by July 1982.

o Pre-Requisite testing of the IIV A C is scheduled to start on April 23, 1982 and will be ready for operation by October 1982.

o All of the control panels are available now for visual inspection including the remote shutdown panel. Only a small number of the plant systems have been completely tested, therefore only certain system will have power applied. Also, some TMI changes are still in the process of being implemented on some of the Control Boards.

o The Annunciator System is scheduled to be completely checked out by April 23, 1982. It will be ready for operation by August 1982.

o THI changes such as Incore Thermocouple displays and Subcooling Monitor displays are not available. Delivery of this equipment is schedeled for November 1982.

o Limited features of the Plant Monitoring Computer are available for inspection. System will be operational by November 1982.

o Since different test are performed in the Control Room, presently the availability of workspace and environment is not in a condition similar to what will be when operating. The Control Room will not become completely available in a fashion similar to normal operations until September or October 1982.
