FAILURE OF TESTABLE CHECK VALVE POSITION INDICATOR, JUSTIFICATION FOR CONTINUED OPERATION Re: Cooper Nuclear Station Your letter dated January 22, 1987 describes circumstances involving the failure to successfully complete a surveillance test of a testable check valve (RHR-A0-68A) located inside primary containment in the "A" loop LPCI injection line. The surveillance test is performed to satisfy the ASME Section XI Inservice Testing requirement that it be fully stroked quarterly.

An air-operated actuator attached to the valve is used to stroke the valve.

The test verifies freedom of movement. Failure of the valve to fully open during an accident could result in reduced emergency core cooling flow and could thereby increase the consequences of the accident.

According to your letter, the valve position indicator used in performance of the test utilizes reed switch sensors attached to the valve. The reed switches operate indicator lamps in the control room to indicate valve position. When the test was performed the position indicator only showed that the valve opened to a partially open position. (One of the two lamps did not go out to indicate fully open.) Such a test result indicates that the valve is possibly incapable of fully opening, but could be the result of a position indicator malfunction. Your letter provides justification for continued operation (JCO) by providing reasonable argument that the test result was due to malfunction of the position indicator and that the valve did indeed stroke fully open. Your argument was based on analysis of the design and operating history of the position indicating system, and is further supported by the fact that an independent position indicator on the air actuator indicated that the actuator stroked fully open. The staff has reviewed the evidence and concludes that the check valve is operable. The staff acknowledges your commitment to take corrective action to repair, replace or possibly modify the position indicator, and possibly revise testing procedures. The Region will followup on these actions.
