SUBJECT: NRC INSPECTION REPORT 50-298/98-22 AND NOTICE OF VIOLATION
Dear Mr. Hom:
From November 16 through December 8,1998, an inspection related to the implementation of
certain actions associated with your engineering strategy was conducted at your Cooper
Nuclear Station reactor facility. The enclosed report presents the scope and results of that
inspection.
This inspection evaluated the completion of Engineering Strategy Action item 3.2.e., the
unauthorized modification review program, developed to aadress deficiencies from prev!ous

escalated enforcement. This was the first inspection of a completed engineering improvement
strategy action item. The inspectors found that your implementation of the program had not
l
l
identified all unauthorized modifications or properly resolved all of the identified unauthorized
modifications. Although the items identified had low individual safety significance, we
concluded that, collectively, they indicated a lack of effective oversight by plant management.
Future inspections will verify your success in applying the lessons teamed from this inspection
to the completion of other strategy items.

A violation is cited in the enclosed Notice of Violation (Notice) and the circumstances
surrounding this violation are described in detailin the enclosed report. Please note that you
are required to respond to this letter and should follow the instructions specified in the enclosed
i
Notice when preparing your response. The NRC will use your response, in part, to determine
whether further enforcement action is necessary to ensure compliance with regulatory
requirements.
In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter, its
enclosures, and your response will be placed in the NRC Public Document Room (PDR).
