Subject: NPpD Response to Inspection Report 50 298/90 25 This letter is written in response to your letter dated August 23, 1990, transmitting inspection Report 50 298/90 25, Therein you identified two (2)

weaknesses in our emergency preparedness program observed during the 1990 annual emergency exercise. The following is our response to the weaknesses noted: Statement of Weakness 298/9025-01 Continuous accountability was not adequately maintained in the CR (simulator). The inspectors noted that the reason for this' appeared to be an organisational problem in that the CRD, who is tasked to supervise accident mitigation activities, was assigned this additional duty. The licensee did not maintain positive access and egress control in the CR-(simulator); therefore, this resulted in a failure to maintain continuous accountability of essential personnel.

Planned Corrective Action The Control Room Director (CRD) will be relieved of the accountability responsibility by security personnel, Schedule of 1molementation This organisational change including required training will be completed by January 31, 1991.

This prevented the timely loading of the same when it was returned to service. The DG was started at 12:38 p.m., but was not sequenced on the bus until 12:46 p.m., that is 8 minutes later. The operators used 8 minutes to discuss how the DG '

would be loaded and the loads required once it was energized. This delay was critical because this DG was the only source of power at that time that could be used for mitigating the acciden >

Other instances of lesser safety sig. tficance are as follows: The shif t supervisor (SS) did not res ognize conditions for entering Technical Specification (TS) 3.7.A.1.1 Applicable TS entry conditions were met when the suppressio4 pool temperature reached 120*F at 9:30 a.m. As a consequence of . sot recognizing this critical condition, the CR team did not Ligin reactor depressurization until directed to do so ly a contingency message (Message 21 C) at 9:45 ,

' Some decisionmakers in the CR and TSC staffs did not realize that the high pressure coolant injection system (HPCI) was unavailable due to the closure of alternating current (AC) powered inboard isolation valve (M0 15). This valve closed prior to the loss of all *

AC power. The TSC staff thought that HPCI was unavailable due to the problem with the steam line differential pressure (d/p) i switches, and they subsequently directed that the leads be lifted at 9:08 The CR operators did not know why HPCI was unavailable until they attempted to start it at 9:58 a.m. The CR and TSC staffs ,

j had not been given the MO 15 valve position as part of the initial conditions of the scenari Thus, part of this may be due to the ,

artificiality of the manner in which the exercise was conducted (i.e., without the use of the simulator in the interactive mode).- ,

Planned Corrective Action ,

These exercise weaknesses, including lessons learned, will be discussed with operators during the Operations Supervisor's weekly briefin5 of the l operating crews. These discussions will include remarks about l

anticipating and preparing for future operator action In addition,.

future emergency preparedness training for operators and decisionmakers will include objectives directed toward effectively recognizing plant
