Gentlemen:

This refers to the inspection conducted by Messrs. W. R. Bennett and G. A. Pick

of this office during the period April 16 through May 31, 1989, of activities

. authorized by NRC Operating License DPR-46 for Cooper Nuclear Station, and to

the discussion of our findings with Mr. G. R. Horn and other members of your

staff at the conclusion of the inspection.

Areas examined during the inspection included operational safety verification,

monthly surveillance and maintenance observations, installation and testing of

modifications, and refueling activities. Within these areas, the inspection

consisted of selective examination of procedures and representative records,

interviews with personnel, and observations by the NRC inspectors. The

inspection findings are documented in the enclosed inspection report.

During this inspection, it was found that certain of your activities were in

violation of NRC requirements. A Notice of Violation has been enclosed with

this letter for the finding involving a failure to follow a safety procedure

concerning tagged-out equipment. A second violation was issued for a failure

to adhere to the Technical Specification operability requirements for the

standby liquid control (SLC) system, because Squib Valve A was inopwrable.

Consequently, you are required to respond to these violations, in ..riting, in

accordance with the provisions of Section 2.201 of the NRC's " Rules of

Practice," Part 2, Title 10, Code of Federal Regulations. Your response should

be based on the specifics contained in the Notice of Violation enclosed with

this letter.

With respect to the violation related to the finding that you operated for

over a year with an inoperable squib valve in the Standby Liquid Control (SLC)

system, we are concerned that this situation occurred because of inadequate

. work instructions in the design change package. While the SLC system is a

backup system used in the event of a failure of the control rod drive system

and operability of the second squib valve was successfully demonstrated, we

consider inadequate station modification work instructions significant. Other

weaknesses in your plant modification process are identified in paragraph 6 of

the enclosed inspection report. Since your response to the Notice of

Violation should address corrective actions with respect to the plant

modification process, we will review your response to determine if further

discussion on this issue is necessary.
