Gentlemen:

This refers to the routine safety inspection conducted by Mr. L. J. Hueter of

this office on April 7-9, 1987, of activities at Lacrosse Soiling Water

Reactor authorized by NRC Operating License No. DPR-45 and to the discussion

of our findings with Mr. Parkyn and other members of your staff at the

conclusion of the inspection and a followup discussion with Mr. Parkyn by

telephone on May 5, 1987.

The enclosed copy of our inspection report identifies areas examined during

the inspection. Within these areas, the inspection consisted of a selective

examination of procedures and representative records, observations, and

interviews with personnel.

During this inspection, certain of your activities appeared to be in violation

of NRC requirements, as specif.'ed in the enclosed Notice. A w.itten respc.nse

is required. Although your plant personnel identified the violation of a

Technical Specification associated with the event described in LER 86-012,

they did not identify an operator error (failure to follow an alarm response

procedure requiring isolation of the tank upon receipt of the high level alarm)

as the root cause of the event nor did they properly address this root cause

in the corrective measures taken. Please reevaluate the LER and address this

indication of management weakness.

In accord $nce with 10 CFR 2.790 of the Commission's regulations, a copy of

this letter, the enclosures, and your response to this letter will be placed

in the NRC Public Document Room.

The responses directed by this letter and the accompanying Notice are not

subject to the clearance procedures of the Office of Management and Budget

as required by the Paperwork Reduction Act of 1980, PL 96-511.
