Gentlemen:

This refers to the routine safety inspection conducted by Mr. K. Ridgway of

this office from February 7,1988 through March 20, 1988, of activities at the

Lacrosse Boiling Weter Reactor authorized by NRC Operating License No. DPR-45

and to the discussion of our findings with Messrs. J. Parkyn and G. Boyd and

- other members of your staff during the course of the inspection.

The enclosed copy of our inspection report identifies areas examined during

the inspection. Within these areas, the inspection consisted of a selective

examination of procedures and representative records, observations, and

interviews with personnel.

During this inspection, certain of your activities appeared to be in violation

of NRC requirements, as specified in the enclosed Notice. A written response

is required.

The violation, which was identified by your personnel and is described in

LER 88-02, involves a failure to maintain the minimum shift crew composition as

required by the plant technical specifications. While we concluded that this

violation in itself is not safety significant due to the fact that the plant

is permanentny shutdown and defueled and the potential for accidents involving

radioactive releases has been significantly decreased, we are concerned with

the lack of attention to responsibilities on the part of the individuals

involved.

In your response to this violation please address this issue as

well as what administrative or management controls you will impose to prevent

recurrence.

In accordance with 10 CFR 2.790 of the Commission's regulations, a copy of

this letter, the enclosures, and your response to this letter will be placed in

the NRC Public Document Room.

The responses directed by this letter and the accompanying Notice are not

subject to the clearance procedures of the Office of Management and Budget as

required by the Paperwork Reduction Act of 1980, PL 96-511.
