This refers to the special safety inspection conducted by Mr. A. S. Gautam and other NRC representatives of this office on January 5-26, 1987, of activities at the Lacrosse Boiling Water Reactor authorized by NRC License No. ORP-45 and to the discussion of our findings with Mr. J. Parkyn at the conclusion of the inspectio During this inspection, one significant deficiency was identified regarding the implementation of your Environmental Qualification (EQ) Program. This finding was classified as a Potential Enforcement / Unresolved Item in Inspection Report No. 50-409/87002 forwarded by our letter dated March 6,1987. In our letter, we stated that the NRC was reviewing this item in regard to possible future enforcement action. That review is now complet Based on our review of this matter, we have concluded that the identified deficiency was a violation of NRC requirements, as specified in the enclosed Notice. A written response is require In accordance with 10 CFR 2.790 of the Commission's regulations, a copy of this letter, the er. closed Notice of Violation, and your response to this letter will be placed in the NRC Public Document Roo The responses directed by this letter and the acccmpanying Notice are not subject to the clearance procedures of the Office of Management and Budget as required by the Paperwork Reduction Act of 1980, PL 96-51 We will gladly discuss any questions you have concerning this issu
