Reactor Trip System - Turbine Trip Low Autostop Oil Pressure Channel II - Inoperable.

This report was initiated by Incident Report 82-422.

f CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 - RX Power 82 % - Unit Load 890 MWe.

DESCRIPTION OF OCCURRENCE:

At 1400 hours0.0162 days <br />0.389 hours <br />0.00231 weeks <br />5.327e-4 months <br />, November 19, 1982, during routine surveillance, the Turbine Trip Low Autostop Oil Pressure Channel II bistable was observe

  • to trip at 44.8 psig instead of 45 psig or greater as required by the Technical Specifications. In accordance with i Technical Specification Action Statement 2.2.1, the channel was
declared inoperable and Limiting Condition for Operation 3.3.1 Action l

7 applied. The channel was placed in the tripped condition at 1410 hours0.0163 days <br />0.392 hours <br />0.00233 weeks <br />5.36505e-4 months <br />. The redundant channels were operable throughout the occurrence.

APPARENT CAUSE OF OCCURRENCE:

i The cause of the setpoint change was mechanical wear in the channel

pressure switch. No problems with the switch had been noted in the past, and the occurrence was assumed to be of an isolated nature.

ANALYSIS OF OCCURRENEE:

The turbine trip causes a direct reactor trip when operating at

, elevated power. The trip provides turbine protection and reduces the severity of resulting transients. Although no credit is taken for the trip in the accident analyses, its functional capability is required to enhance the overall reliability of the Reactor Trip System.

Since the redundant channels were operable, the event involved no risk to the health and safety of the public. The occurrence constituted operation in a degraded mode permitted by a limiting condition for operation and is reportable in accordance with Technical Specification 6.9.1.9b.

Limiting Condition for Operation 3.3.1 Action 7 requires:

With the number of operable channels one less than the total number of channels, power operation may proceed until performance of the next required channel functional tect provided the inoperable channel is placed in the tripped condition within one hour.

CORRECTIVE ACTION:

As noted, the channel was placed in the tripped condition, in compliance with the action statement. The pressure switch was replaced and calibrated to the required setpoint. Autostop Oil Low Pressure Channel II was satisfactorily tested, the channel was declared operable, and at 1635 hours0.0189 days <br />0.454 hours <br />0.0027 weeks <br />6.221175e-4 months <br />, November 10, 1982, Limiting Condition for Operation Action 7 was terminated.
