No. 24 Steam Generator Pressure Channel 4 - Inoperable.

This report was initiated by Incident Report 82-008.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 - Rx Power 97% - Unit Load 1080 MWe DESCRIPTION OF OCCURRENCE:

On January 10, 1982 the operator noticed that No. 24 Steam Generator Pressure Channel 4 had failed high. At 0300 hours0.00347 days <br />0.0833 hours <br />4.960317e-4 weeks <br />1.1415e-4 months <br /> the pressure channel was declared inoperable and Action Statement 3.3.2 Action 14 was entered.

This occurrence constituted operation in a degraded mode in accordance with Technical Specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE :

The reason No. 24 Steam Generator Pressure Channel 4 failed high was the. ventilation damper stuck open, freening the pressure sensing line.

ANALYSIS OF OCCURRENCE:

Technical Specification 3.3.2 Action 14 requires:

With the number of operable channels one less than the

! total number of channels, operation may proceed until performance of the next required channel functional test, provided the inoperable channel is placed in the tripped condition iwithin one hour.

C,ORRECTIVE ACTION:

The ventilation damper was freed and exercised, then closed.

The pressure sensing line was thawed, tested, and returned to service. At 0325 hours0.00376 days <br />0.0903 hours <br />5.373677e-4 weeks <br />1.236625e-4 months <br />, January 10, 1982, Action Statement 3.3.2 Action 14 was terminated.

Design Change Request (DCR) 2SC-0160 was completed July 11, 1983. l This DCR installed temperature monitoring in the north and south penetration areas with associated alarms (at 390F) on the auxiliary annunciator in the control room. A similar design change (DCR ISC-0159) is being incorporat.ad on Salem Unit No. 1. I FAILURE DATA:
