ENCLOSURE 1 NOTICE OF VIOLATION Georgia Power Company Docket No. 50-425 Vogtle Unit 2 Licease No. CPPR-109 f

During the Nuclear Regulatory Commission (NRC) inspection conducted on February 1-4, 1988, a violation of NRC requirements was identified. In accordance with the "General Statement of Policy and Procedur? for NRC Enforcement Actions," 10 CFR Part 2. Appendix C (1988), the violation is listed ,

below:

10 CFR, Appendix B, Criterion V requires that activities affecting quality shall _ be prescribed by documented procedures and shall be accomplished  !

in accordance with these procedures. In Section 17.1.2 of the SAR, the l licensee committed to a quality assurance program which will comply with the quality assurance criteria of 10 CFR 50, Appendix B.

! Procedure 25733-C, Revision 4 "High Potential Testing Cable", Paragraph

, 4.15 states that, if leakage current increases with tine, extend the test period to 15 minutes. Procedure 25733-C, Revision 4, Paragraph 4.11 .

states that the test voltage will be 56kV DC for cable bus in a SkV rated system. Procedure 25733-C, Revision 4, Paragraph 3.3 states that, if work ,

is safety-related, notify QC prior to start of work. Procedure 25733-C,

  • Revision 4, Paragraph 3.2 requires the persons performing the test to detennine the safety classification of the equipment and check the appropriated classification on the Completion Sheet. '

Contrary to the above, Procedure 25733-C, Revision 4 was not always '

followed as described in the following four cases. When testing cable No. 2AA0204EA (feeder to nuclear service cooling water pump A motor) on l September 15, 1987, the leakage current for Phase C conductor was slowly  ;

increasing, but the test was stopped at five minutes. When testing the cable bus from emergency generator A, on July 30, 1987, a test voltage equal to 36kV DC was applied. For 30 of the 42 required tests, Quality

. Control was not notified prior to start of work. For 21 of the 42 required tests the safety class lfication was not properly checked on the completion sheet.

This is a Severity Level IV violation (Sepplement II).

Pursuant to the provisions of 10 CFR 2.201, Georgia Power Company is hereby  :

required to submit a written statement or explanation to the Nuclear Regulatory i

Commission, ATTN: Document Control Desk, Washington, DC 20555, with a copy to the Regional Administrator, Region II, ano a copy to the NRC Resident i Inspector, Vogtle Unit 2, within 30 days of the date of the letter transmitting 8803250099 880318 PDR ADOCK 05000425 ,

Georgia Power Company 2 Docket No. 50-425 Vogtle Unit 2 License No. CPPR-109 this Notice. This reply should be clearly marked as a "Reply to a Notit.e of Violation" and should include [for each violation]: (1) admission or denial of the violation, (2) the reason for the violation if admitted, (3) the corrective steps which have Nen taken and the results achieved, (4) the corrective steps which will be taken to avoid further violations, and (5) the date when full compliance will be ach!eved. Where good cause is shown, consideration will be given to extending the response time. If an adequate reply 10 not received within the time specified in this Notice, an order may be issued to show cause why the license should not be modified, suspended, or re,oked or why such other action as may be proper should not be taken.

Security or safeguards information should be submitted as an enclosure to facilitate withholding it from public disclosure as required by 10 CFR 2.790(d) or 10 CFR 73.21.
