ENCLOSURE 1 NOTICE OF VIOLATION Georgia. Power Company Docket Nos. 50-424 and 40-425 Vogtle, Units 1 and 2 License Nos. NPF-68 and NPF-79 EA 89-63 During the Nuclear Regulatory Commission (NRC) inspection conducted on February 18 - March 17, 1989, two violati3ns of NRC requirements were identified. In accordance with the " General Statement of Policy and Procedures for NRC Enforcement Actions" 10 CFR Part 2, Appendix C (1988), the violations are listed below.

A. Technical Specification 3.5.2 requires that with the unit in Modes 1, 2, and 3, two independent Emergency Core Cooling System subsystems be

- operable with each _ subsystem comprised of one operable centrifugal charging pump, one operable Safety Injection Pump, one operable residual heat removal system (RHR) heat exchanger, one operable RHR pump, and an operable flow path capable of taking suction from the refueling water storage tank on a Safety Injection signal and semi-automatically transferring suction to the containment emergency sump during the recirculation phase of operation.

Contrary to the above, on March 9,1989, with Unit 2 in Mode 3, two independent Emergency Core Cooling System subsystems were not operable in that operable flow paths did not exist when RHR valves 1205-U4-027 and 1205-U4-226, which were required to be locked closed, were opened. The opening of these valves provided an additional path for flow which degraded the safety capability of both subsystems to provide the required accident analysis postulated flowrates to the core. This condition existed from approximately 3:50 a.m. EST until 6:20 p.m. EST. Upon discovery of this condition, the valves were immediately restored to their required position.

This is a severity level IV violation (Supplement I). 4 This violation applies to Unit 2 only.

B, . 10 CFR Part 50, Appendix B, Criterion X, and the licensee's accepted QA program, Final Safety Analysis Report (FSAR) Section 17.2.10 requires that a program for inspection of activities affecting quality shall be established and executed by or for the organization performing the l activi ty to verify conformance with the documented instructions, I procedures, and drawings for accomplishing the activity. ANSI N45.2.8, j 1975 committed to in FSAR chapter 17.2.10 states that checks and j inspections shall be performed to verify the operational readiness and  !

completeness of components and systems. Plant procedure, Work Planning  !

and Hold Point Assignment (85301-C), implements the inspection activities l by providing for the establishment and control of Quality Control hold points in work activities. Procedure step 5.1.4 requires hold points for torqueing and alignment items.

, Contrary to the above, on March 6,1989, the inspector identified that l checks and inspections were not performed to verify the operational readiness and completeness of the mechanical portions of a safety-related system while conducting repairs on the Containment Spray pump "B" room cooler fan. Plant procedure 85301-C was not followed in that mechanical Quality Control hold points were not established in maintenance work order 18901156 for torqueing and alignment of the bearings and belt on the cooler fan. l This is a severity level IV violation (Supplement I).

This violation applies to Unit 1 only.

Pursuant to the provisions of 10 CFR 2.201, Georgia Power Company is hereby required to submit a written statement or explanation to the Nuclear Regulatory Commission, ATTN: J Document Control Desk, Washington, DC 20555, with a copy I to the Regional Administrator, Region II, and a copy to the NRC Resident Inspector, Vogtle, within 30 days of the date of the letter transmitting this Notice. This reply should be clearly marked as a " Reply to a Notice of  ;

Violation" and should include for each violation: (1) admission or denial of l the violation, (2) the reason for the violation if admitted, (3) the corrective steps which have been taken and the results achieved, (4) the corrective steps which will be taken to avoid further violations, and (5) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time. If an adequate reply is not received within the time specified in this Notice, an order may be issued to show cause why the license should not be modified, suspended, or revoked or why such other action as may be proper should not be taken.
