NOTICE OF VIOLATION Southern Nuclear Operating Company. Inc.
Docket No. 50-425 Vogtle Electric Generating Plant License No. NPF-81 Unit 2-During an NRC inspection conducted March 8.1998, through April 18. 1998.
violations of NRC requirements were identified.
In accordance with the
" General Statement of Policy and Procedure for NRC Enforcement Actions."
(NUREG 1600), the violations are listed below:
A.
Technical Specification 5.4.1.a requires that written procedures be implemented for the activities identified in Appendix A of Regulatory Guide (RG) 1.33. Revision 2. February 1978.

Appendix A. requires that procedures for maintenance activities be implemented.
Procedure PT-V-605. " Liquid Penetrant Examination Procedure." Revision i
- 3. paragraph 9.2 states in part that "The examination area and the adjacent areas within at least 1 inch shall be dry and free of any dirt, grease, lint water, scale, welding flux. weld spatter, oil or other 1
extraneous matter that could interfere with the examination."
Contrary to the above, on April 1, 1998, Procedure PT-V-605, was not properly implemented, in that. liquid penetrant pre-examination cleaning

for safety injection system weld No. 21204-122-6. was inadequate. A tenaciously adhering white deposit approximately 8 x y inch was left at the toe of the weld after precleaning and prior to the ap)1ication of the penetrant. This deposit was of a size that could masc relevant rejectable indications.
This is a Severity Level IV violation (Supplement I).
B.
License Condition (E) of the Vogtle Operating License, dated March 17, 1997, states, in part that Southern Nuclear shall fully implement and maintain in effect all provisions of the Commission-approved physical security, guard training and qualification, and safeguards contingency
_ plans....and to the authority of 10 CFR 50.90 and 10 CFR 50.54(p).
Vogtle Electric Generating Plant Physical Security and Contingency Plan.
Revision 34. Section 3.2.3. states that procedures exist to document the structure of the security organization and to detail the duties of security officers and others responsible for security.
Procedure 00652-C, " Personnel Escort-Duties and Responsibilities."
Revision 5. Section 4.8. states that an escort shall maintain control of and visual contact with a visitor.

Contrary to the above, on March 30 and March 31. 1998. an escort was observed as not maintaining control and visual contact of a visitor inside the Unit 2 Train B emergency diesel generator building.
This is a Severity Level IV violation (Supplement III).
Pursuant to the provisions of 10 CFR 2.201. Southern Nuclear Operating Company. Inc. is hereby required to submit a written statement or explanation to the U.S. Nuclear Regulatory Commission. ATTN:
Document Control Desk.
Washington. D.C. 20555. with a copy to the Regional Administrator. Region II.
and a copy to the NRC Resident Inspector. Vogtle, within 30 days of the date of the letter transmitting this Notice of Violation (Notice).
This reply should be clearly marked as a " Reply to a Notice of Violation" and should include for each violation:
(1) the reason for the violation, or, if contested, the basis for disputing the violation. (2) the corrective steps that have been taken and the results achieved. (3) the corrective steps that will be taken to avoid further violations, and (4) the date when full compliance will be achieved.
If an adequate reply is not received within the time specified in this Notice, an order or demand for information may be issued as to why the license should not be modified suspended, or revoked, or why such other action as may be proper should not be taken.
Where good cause is shown, consideration will be given to extending the response time.
Because your res)onse will be placed in the NRC Public Document Room (PDR). to
'the extent possi)le, it should not include any personal privacy. 3roprietary, or safeguards information so that it can be placed in the PDR wit 1out redaction.
However, if you find it necessary to include such information, you should clearly indicate the specific information that you desire not to be placed in the PDR and provide the legal basis to. support your request for withholding the information from the public.
