NOTICE OF VIOLATION Southern Nuclear Operating Company. Inc.

Docket Nos. 50-424 and 50-425 Vogtle Electric Generating Plant License Nos. NPF-68 and NPF-81 Units 1 and 2 During an NRC inspection conducted June 28. 1998, through August 15. 1998, violations of NRC requirements were identified.

In accordance with the

" General Statement of Policy and Procedures for NRC Enforcement Actions."

(NUREG 1600). the violat wns are listed below:

A.

10 CFR 50. Appendix B. Criterion XVI, " Corrective Actions." states, in part, that measures shall be established to assure that conditions adverse to quality are promptly identified and corrected.

In the case of significant conditions adverse to quality the measures shall assure that the cause of the condition is determined and corrective action taken.to preclude repetition, j

Contrary to the above. the corrective actions-taken in response to violations associated with entry into Technical Specification (TS)

Limiting Conditions for Operation (LCO) for maintenance and surveillances activities were inadequate to preclude repetition.

Failures to document entries into applicable TS LCOs are identified in NRC Inspection Reports 50-424. 425/98-04; 50-424. 425/98-02: 50-424, 425/97-10: and: 50-424. 425/97-05.

Subsequently, on July 23, 1998. Unit 1 TS LCO 3.3.6. " Containment Ventilation Isolation Instrumentation." for inoperable radiation monitors during performance of surveillance procedure 14420-1

" Solid State Protection System and Reactor Trip Breaker Train A Operability Test." Revision 30, was not documented in the control room log.

This is a Severity Level IV violation (Supplement I).

B.

License Condition (E) of the Vogtle Operating License, dated March 17.

1997, states, in part, that Southern Nuclear shall fully implement and maintain in effect all provisions of the Commission-approved Physical Security. Guard Training and Qualification, and Safeguards Contingency Plans...and to the authority of 10 CFR 50.90 and 10 CFR 50.54(p).

Vogtle Electric Generating Plant Physical Security and Contingency Plan.

1 Revision 34. Section 3.2.3 states in part. that procedures exist to document the structure of the security organization and to detail the duties of security officers and others responsible for security.

I Procedure 00652-C, " Personnel Escort-Duties and Responsibilities."

Revision 5. Section 4.8.- states that an escort shall maintain control of and visual contact with a visitor.

Contrary to the above, on July 16. 1998, an escort was observed as not maintaining control and visual contact of three visitors inside the Electric Boiler building.

This is a Severity Level IV violation (Supplement III).

C.

Technical Specification 5.7.2 High Radiation Area requires each locked high radiation area, as defined in 10 CFR 20. shall be provided with locked or continuously guarded doors to prevent unauthorized entry.

Contrary to the above, on July 27, 1998. a posted locked high radiation area in the Unit 2 Auxiliary Building Room 2-AB-A-70, was found unlocked and not continuously guarded to prevent unauthorized entry.

This is a Severity Level IV violation (Supplement IV).

The NRC has concluded that information regarding the reasons for the violations, the corrective actions taken and planned to correct the violations and prevent recurrence is already adequately addressed on the docket in this inspection report.

However, you are required to respond to the provisions of 10 CFR 2.201 if the description therein does not accurately reflect your corrective actions or your position.

In that case or if you chose to respond, clearly mark your response as a " Reply to a Notice of Violation." and send it to the U.S. Nuclear Regulatory Commission. ATTN:

Document Control Desk. Washington, D.C. 20555 with a copy to the Regional Administrator. Region II, and a co]y to the NRC Resident Inspector at the facility that is the subject of t1is Notice, within 30 days of the date of the letter transmitting this Notice of Violation (Notice).
