' Subject: Confirmatory Action Letter 87-05 -- Violation of Primary Containment

Due to Blocked-Open Vacuum Breakers

J We ha'.e reviewed your letter, dated May 7,1987, describing your corrective

actions pursuant to the subject event, your investigation thereof, and the

,

related NRC Region I special inspection (50-219/87-16). Pursuant to the

! telephone conversation on May 8,1987, between Mr. Samuel Collins of my staff

and Mr. Peter Fiedler of your staff, it is our understanding that you will (1)

complete those actions as stated in the letter, and (2) also take the following

actions prior to restart from the current maintenance outage:

! a. Prohibit site individuals from performing any RTR or ISR functions,

j including temporary variations or any other RTR/ISR duties, until the

individual has successfully completed the retraining described in your

letter.

b. Brief NRC Region I management on the following items: t

personnel of all open temporary variations, including any resultant

corrective actions. I

c. Brief NRC Region I management regarding actions GPUN has taken to assure

itself that the safety review process is working adequately in all areas

(not limited only to temporary variations) to ensure a high level of

nuclear safety. GPUN's actions in this regard will include a corporate-

level re-review of the August 1986 and January 1987 temporary variation

and safety review assessments to determine if any immediate corrective

actions are warranted. GPUN's evaluation will include an assessment of

the potential for adverse impact from past safety review problems,

d. Have management reiterate to each GSS and STA the proper organizational

relationship and independence of designated overview functions.
