During the time period of April 9 through June 8,1990, the NRC conducted a

licensed operator requalification program evaluation at'the Oyster Creek

Nuclear Generating Station. The evaluation was conducted in.accordance with

Revision ~ 5 to NUREG-1021, " Operator Licensing Examiner Standards," specifi-

cally, ES-601, " Administration of NRC Requalification Program Evaluations."

The evaluation had two purposes: (1) to evaluate individual ^ licensed operator

'

competency; and, (2) to evaluate the effectiveness of the licensed operator

requalification training program at Oyster Creek.'

. As a result of this evaluation, the Oyster Creek licensed operator requalifi-

cation. program has been rated as satisfactory; however, several significant

weaknesses were noted. The NRC results indicate that of the'21 licensed operators

,

examined, 5 of the 12 reactor operators failed at laast one part of the

examination. All 9 senior reactor operators and all 4 crews were determined to

be satisfactory. Section 2 of the attached report contains the details of the '

program evaluation. t

The Oyster Creek licensed operator requalification program contained the  ;

'following significant weaknesses: (1) failure to establish and implement- r

l appropriate standards of' performance in the evaluation. process; (2)' failure to

l- identify poor quality procedures and utilize the requalification program to -

t

l improve the quality of the operating procedures; and,'.(3) failure to evaluate

j. Reactor Operators (R0s) in all positions allowed by- their licenses during the _

l

facility administered annual operating tests. The enclosed report provides the  !

details of these weaknesses.

GPUN letter to the NRC, dated July 26, 1990, Attachment 5 of the enclosed i

report, is the GPUN evaluation of' the Oyster. Creek licensed. operator requali-

fication program. .The evaluation also includes the corrective actions for .

.

weaknesses noted by the Oyster Creek training organization. The-NRC has

reviewed the evaluation and has the following concerns: .(1) a root-cause  ;

analysis that identifies why the noted weaknesses occurred was not provided;

' (2) the evaluation did not addrcss procedure quality;:(3)'the evaluation did ,

not address the R0 rotation practices during the annual operating examination; e

and, (4) the corrective actions did not include provisions for upgrading ,

evaluator ability to conduct objective evaluations and critiques, f

Accordingly, we request that you respond, in writing, within 30 days of the

receipt of this letter confirming the corrective actions committed to during

the interim exit meeting on April 24, 1990, and the final exit meeting on June

26, 1990. Additionally, the response should address the above noted weaknesses i

and concerns relative to the evaluation provided by letter to the NRC dated

July 26, 1990. Section 7 of the enclosed report contains the NRC's under-

standing of your facility's commitments, made to date, to correct the weak- ,

nesses noted relative to the requalification program.

In accordance with 10 CFR 2.790 of the Commission's regulations, a copy of this

letter and its enclosure will be placed in-the NRC Public Document Room. The

response requested by this letter is not subject to the clearance procedures of

the Office of Management and Budget as required by the Paperwork Reduction Act

of 1980, Public Law No.96-511.
