50-456/81-10 and 50-457/81-10 NRC Docket Nos. 50-456/457 Reference (a): October 14, 1981, letter from C. E. Norelius to Cordell Reed.

Dear Mr. Keppler:

Reference (a) contained the report of an inspection conducted by Mr. C. M. Erb of activities at Braidwood Station on September 1-3, 1981. During that inspection, certain activities were found to be in noncompliance with NRC requirements. Attachment A to this letter contains Commonwealth Edison's response to the Notice of Violation.

To the best of my knowledge and belief the statements contained herein and in the attachment are true and correct. In some respects these statements are not based upon my personal know-ledge but upon information furnished by other Commonwealth Edison employees and contractors. Such information has been reviewed in accordance with Company practice and I believe it to be reliable.

ATTACHMENT A Response to Notice of Violation Violation:

10 CFR 50, Appendix B, Criterion V states in part, " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, . . .and shall be accomplished in accordance with these instructions, procedures or drawings.

S&L Specification 2722, Form 1700-T and ABD Procedure No. 5, Revision 1, require (1) torch cut holes must be reamed to size, (2) bent bolts up to 100 from perpendicular be cold straightened and nuts be hand tight, and-(3) workmanship insuring bearing be attained with shims.

Contrary to the above, the following conditions were found at the 376 foot level outside containment:

Unit #1 Beam No. Size Problem 100281 14" Torch cut hole in flange of beam 100283 14" Bent bolts A406B1 16" Bent bolts, loose nut Unit #2 400681 16" Bent bolts, loose nut A42B32 14" Bent bolts, less than full bearing A606B1 15" Bent bolts, less than full bearing Corrective Action Taken:

A. Commonwealth Edison NCR #313 was written on 9/02/81 to disposition the torch cut holes in Beam 1002Bl. A plate washer will be welded to the flange in accordance with StL Standard Specification BY/BR/CEA.

B. Bent bolts on Beams 100283, A406B1, A42B32 and A60681 were straightened using Gust K. Newberg Procedure #38.

C. Shim plates were added between bearing plates and beams to ensure full bearing.

Corrective Action Taken to Prevent Reaccurrence:

The beams as originally installed by American Bridge were acceptable at the time of installation but were removed by another contractor to install pipe and equipment and were subsequently never reinspected'upon reinstallation. A letter has been issued to all contractors to gain CECO permission prior to disassembling any piece of installed equipment or structure.

A sample reinspection of A-307 bolted connections will be performed by the independent testing contractor to insure that this problem does not occur anywhere else.

Date When Full Como11ance Will Be Achieved:

A. The items identified by the inspection report were corrected prior to the Exit Meeting on September 3, 1981.

B. It is expected that the sample reinspection will be-completed by Janauary 1, 1982.
