SUBJECT: NOTICE OF VIOLATION

(NRC INSPECTION REPORT NOS. 50-259/88-05, 50-260/88-05, AND

50-296/88-05)

This refers to the Nuclear Regulatory Commission (NRC) inspection conducted by

G. L. Paulk on March -1-31,1988. The inspection included a review of activi-

ties authorized for your Browns Ferry facility. At the conclusion of the

inspection, the findings were discussed with those members of your staff

identified in the enclosed inspection report.

Areas examined during the inspection included operational safety, maintenance

observation, surveillance testing observation, reportable occurrences, plant '

operations review committee (PORC), restart testing and Q-List. Within these

areas, the inspection consisted of selective examinations of procedures and

representative records, interviews with personnel, and observation of activi-

ties in progress.

The inspection findings indicate certain activities appeared to violate NRC

requirements. The violations, references to pertinent requirements, and

elements to be included in your response are described in the enclosed Notice

of Violation.

Violation D described in the enclosed Notice is similar to a violation con-

tained in the Notice sent to you by our letter dated April 21, 1986. Because

"similar violations," as described in the NRC Enforcement Policy are of

significant concern to the NRC, please give particular attention,in your

response to the identification of the root cause of this problem and your

corrective action to prevent recurrence.

Two unresolved items were identified in the inspection report. The first

matter ralates to proper designation of alternate Plant Operations Review

Committee (PORC) chairmen in your Plant Managers Instruction 7.1 which are

other than those stated in Technical Specification 6.5.1.2.a. The second

matter relates to Q-list programmatic deficiencies which require further

evaluation by your staff to ensure that the Q-list can be relied upon in

determining safety-related and important to safety equipment.

Title 10, Code of Federal Regulations, a copy of this letter.and its enclosures

will be placed in the NRC Public Document Room.

The responses directed by this letter and the enclosures are not subject to the-

clearance procedures of the Office of Management and Budget issued under the

Paperwork Reduction Act of 1980, PL 96-511.
