Vogtle 1 License No..CPPR-108 During the. Nuclear Regulatory Commission (NRC) inspection conducted on August 12 -

October 15, 1986, the following violations of NRC requirements were identified.

,The violations involved corrective action and preoperational testing programs.

In accordance with the " General Statement of- Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C, the-violations are listed below:

1. 10 CFR 50, Appendix B,. Criterion XVI, and the. accepted Quality Assurance (QA) Program (FSAR 17.1.16) collectively require that conditions whi.ch are adverse to ' quality be documented and that corrective action be taken to preclude repetition. - GPC-QA Audit No. OP11/16-86/77 issued on July 9,1986, j identified a . weakness in that the Operations Department did not have a

' formal procedurally controlled fuel handling . crane operator training and qualification program. Corrective action detailed in the audit-report was r that the 'l informal" existing program would 'be procedurally controlled and implemented prior to initial receipt of fuel.

Contrary to the above, on September 4, 1986, an NRC. inspection identified that the fuel handling crane operator training program was not a formal procedurally controlled training and qualification program prior to receipt -

and handling of fuel.

This is a Severity Level IV violation (Supplement II).

2. 10 CFR 50, Appendix B,- Criterion V, and the accepted QA Program (FSAR 17.1.5) collectively require that activities affecting quality be prescribed
by documented instructions, procedures, or drawings, of a type appropriate to the circumstance and be accomplished in accordance with these instructions, procedures, or drawings. Construction Acceptance Test (CAT) procedure

.- . No. CAT-M-04 " Air Operated Valves" details the licensee's testing method for

complying with Regulatory Guide 1.68.3 "Preoperational Testing of Instrument
and Control Air Systems". Section-C.8.

Georgia power Company 2 Docket No. 50-424 Vogtle 1- License No. CPPR-108 Verification that the valve moves to its Fail-Safe position by simulating a gradual loss of instrument air pressure is specified in Step 6.2.12 and for a sudden loss of instrument air pressure is specified in Step 6.4.1.

Contrary to the above, on September 19, 1986, an NRC inspection identified that air operated valves with Bailey Controllers were not tested to assure that the valve moves to its fail-safe position on a sudden loss of instrument air pressure. The failure to properly test the valves is attributed to step 6.4.1 of CAT-M-04 not properly detailing a method for removing instrument air (by simulating a sudden air line break) from these air operated valves.

This is a Severity Level IV violation (Supplement II).

Pursuant to the provisions of 10 CFR 2.201, Georgia Power Company is hereby required to submit to this Office within 30 days of the date of this Notice a written statement or explanation in reply including for each violation: (1) admission or denial of the violation; (2) the reason for the violation if admitted; (3) the corrective steps which have been taken and the results achieved; (4) the corrective steps which will be taken to avoid further viola-tions; and (5) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
