Dear Mr. Riggin:

During completion of the quas .erly report , it was noted that there vere violatiens of outfall 101 requirements not previously reported in accordance with section II.B.2 of the Maryland State Discharge 3 Permit No. Th-DP-0187A. Infomation concerning these violations is presented below:

1) On October 12, free available chlorine was determined to be ,

. 0 7 mg/1. The chlorine level was reduced to 40.1 mg/l on October 1b. An overadjustment of the pump stroke or the chlorine pump resulted in a slight increase over the pemit requirement of 0.5 mg/1.

2) On November 21, L reading of 15 mg/l free available chlorine -

was recorded. The cause was identified to hav' resulted from an improp'- hatched chlorine storage tank. srrective action v- Miated and the concentration reduced to <0.1 mg/l on the 2 k _f Ebeember. To prevent recurrence of thia incident all technicians will be retrained on the procedure of batching chlorine c ncentrate tanks.

3) On October 29, a ec.11 form reading of 3h5 mpn/100 mi s as deter-cined. This is a violation of section I.D. of the Mary)and State Discharge Permit No. Th-DP-0187A. The event was believed to be caused by an unusually high flow to the r,ewage plant soon after the start of a refueling outage. This surge substantially in-creased tne flow in the chlorine contact tank, which, in turn, reduced the chlorine contact time. This contact time was too short to provide proper disinfection. b) On October 22, the tevnge treatment plant discharge suspended solids- concentration was determined to be h8.8 mg/1, primarily due to exceptionally high flow. This is grt eter than the i allowable h5 mg/l authorized by section I.D. of the reference permit. In addition, the monthly average suspended solide i concentration was calculated to be 315 mg/1, above the permit specification of 30 mg/1. l To insure compliance with the reporting requirements of the Maryland State Discharge Permit, all technicians have been reinstructed to inform the i appropriate supervisor (s) lor all uut-of-specification chemistry parameters. -l Sincerely yours, L'. ..
